コミュニケーションが可能なALS患者を対象に意思伝達ツールを5回練習し,心理変化と文字入力スピードを経時的に観察することで意思伝達ツールの導入に対する患者の客観的評価を調査する.
文字入力練習は青木らの先行研究により,600文字程度の練習で上達する(青木,2005)ことを参考とし,今回は1回200文字の練習を5回行う事とした.この練習による文字入力の上達と心理変化を観察するために,練習回ごとに調査する.

We will investigate patients' objective evaluation of the introduction of the communication tool by practicing the communication tool five times with ALS patients who are able to communicate, and observing their psychological changes and text input speed over time.
Regarding character input practice, based on previous research by Aoki et al., which showed that students can improve by practicing around 600 characters (Aoki, 2005), we decided to practice 200 characters at a time five times. Examine each practice session to monitor progress and psychological changes.

1)訓練プログラムを実施する部屋へ案内 2)視線入力装置が装着されたコンピューターの正面に座る 3)患者の目とコンピューターの画面までの距離を45~90㎝にする 4)器機のセッティングとして,画面に映る移動する点を目で追うキャブレーションを行う 5)文書作成ソフトを立ち上げ,仮想キーボードを画面上に表示する 6)仮想キーボードを用いて文字入力を行う.視線入力訓練の題材はアンデルセン童話より計200文字の文章を抜粋した物を使用する.訓練時間は200文字の入力が完了次第終了とするが最長で60分までとする.

①18歳以上の患者 ②ALSまたは,ALS疑いと診断されている患者 ③脳神経内科に通院中の患者 ④脳神経内科の担当医が計5回の介入プログラムに参加可能な病状であると判断した患者 ⑤本人が意思伝達ツールの使用を希望し,文書で同意が得られる患者 ⑥1時間程度の自立座位保持が可能な患者 ⑦コンピューターまたはスマートフォンなどのデジタル機器の操作が可能な患者 ⑧文字や音声によるコミュニケーションが可能な患者 ⑨認知障害のない患者(MMSE23点以上,またはHDS-R15点以上) ⑩家族等の協力が得られる患者

データ収集および参加者の背景

研究参加者の背景情報は、電子カルテから収集した。収集項目は、性別、生年月、家族構成、職歴、日常生活におけるパソコンおよびスマートフォンの使用経験、ALSの発症年月、改訂ALS機能評価尺度(ALS Functional Rating Scale-Revised:ALSFRS-R)スコア、ならびに入院・外来の別であった。

症例1
症例1は64歳の男性で、2024年3月にALSと診断され、同年4月に本研究へ登録された。初回介入時には自立歩行が可能であり、言語によるコミュニケーションに支障はなかった。デスクワークを中心とする職業に従事した経験があり、パソコン操作に習熟し、タッチタイピングが可能であった。訓練セッションにおいて特別な調整を必要とすることなく、訓練プログラムを完了した。

症例2
症例2は72歳の女性で、2024年2月にALSと診断され、同年6月に本研究へ登録された。初回介入時には自立歩行が可能であったが、球麻痺に伴う構音障害を認めた。パソコンは主に娯楽目的で使用しており、タッチタイピングは困難であった。訓練セッション中に機器の設定で一部困難が生じたものの、訓練プログラムを完了することができた。

症例3
症例3は78歳の男性で、2024年6月にALSと診断され、同年8月に本研究へ登録された。初回介入時には自立歩行が可能であったが、進行性球麻痺に伴う構音障害を認めた。長年にわたり業務でパソコンを使用しており、タッチタイピングが可能であった。

訓練準備時、両眼で視線入力装置を使用するためのキャリブレーションに相当な時間を要した。そのため、設定を単眼による視線入力へ変更した。この調整によって円滑な文字入力が可能となり、以降のすべてのセッションで単眼による視線入力を使用した。

Data Collection and Participant Characteristics Background information on the study participants was collected from their electronic medical records. The variables included sex, year and month of birth, family composition, occupational history, experience using computers and smartphones in daily life, date of ALS onset, ALS Functional Rating Scale-Revised (ALSFRS-R) score, and inpatient or outpatient status. Case 1 Case 1 was a 64-year-old man who was diagnosed with ALS in March 2024 and enrolled in the study in April 2024. At the time of the initial intervention, he was able to walk independently and had no difficulty with verbal communication. He had worked in a desk-based occupation, was proficient in computer use, and was able to touch-type. He completed the training program without requiring any special adjustments during the sessions. Case 2 Case 2 was a 72-year-old woman who was diagnosed with ALS in February 2024 and enrolled in the study in June 2024. At the time of the initial intervention, she was able to walk independently but exhibited dysarthria associated with bulbar palsy. She had used a computer primarily for recreational purposes and had difficulty touch-typing. Although some difficulties arose in configuring the equipment during the training sessions, she was able to complete the training program. Case 3 Case 3 was a 78-year-old man who was diagnosed with ALS in June 2024 and enrolled in the study in August 2024. At the time of the initial intervention, he was able to walk independently but exhibited dysarthria associated with progressive bulbar palsy. He had used computers for many years in a professional capacity and was able to touch-type. During preparation for the training, calibration of the eye-tracking input device for binocular use required considerable time. The configuration was therefore changed to monocular eye tracking. This adjustment enabled smooth text input, and monocular eye tracking was used in all subsequent sessions.

参加者の流れ/Participant flow

訓練プログラムおよび実施環境

訓練プログラムは約1か月間隔で実施する計5回のセッションで構成し、各参加者の受診状況に応じて日程を調整した。研究登録時に入院していた参加者については、入院中に初回セッションを実施した。外来で参加した者については、退院後最初の外来受診時に研究手続きを開始した。

各訓練セッション中に入力された文字数を測定した。各セッション内にあらかじめ設定した1分間の測定を5回実施し、1分当たりの平均入力文字数を算出した。

訓練セッションは、院内の静かな個室で実施した。各参加者は、視線入力装置を備えたコンピュータの前に着座した。参加者の眼とコンピュータ画面との距離は、40~90 cmの適切な範囲に保った。

訓練には、Tobii Technology社製のPC Eye5 12)を使用した。各セッションの開始時に、参加者の視線を正確に検出できるよう装置のキャリブレーションを行った。先行研究の手順に従い、参加者にはWindows標準のスクリーンキーボードを使用して、200文字の課題文を入力するよう求めた。各セッションは、参加者が200文字の入力課題を完了した時点、または実施時間が60分を超えた時点で終了した。

Training Program and Setting

The training program consisted of five sessions conducted at approximately 1-month intervals, with the schedule adjusted according to each participant's hospital visits. For participants who were hospitalized at the time of enrollment, the first session was conducted during hospitalization. For those participating on an outpatient basis, the study procedures were initiated at their first outpatient visit after discharge.

The number of characters entered was measured during each training session. Character input was measured during five predetermined 1-minute periods within each session, and the mean number of characters entered per minute was calculated.

The training sessions were conducted in a quiet private room within the hospital. Each participant sat in front of a computer equipped with an eye-tracking input device. An appropriate distance of 40-90 cm was maintained between the participant's eyes and the computer screen.

A PC Eye5 device manufactured by Tobii Technology12) was used for the training. At the beginning of each session, the device was calibrated to accurately detect the participant's gaze. Using the standard Windows on-screen keyboard, participants were instructed to enter a target text of 200 characters, consistent with the procedure used in previous studies. Each session was terminated when the participant completed the 200-character entry task or when the session duration exceeded 60 minutes.

筋萎縮性側索硬化症評価質問票40(ALSAQ-40)

主要評価項目は、ALS患者の生活の質(quality of life:QOL)であった。ALS患者の疾患特異的QOL評価尺度である筋萎縮性側索硬化症評価質問票40(Amyotrophic Lateral Sclerosis Assessment Questionnaire-40:ALSAQ-40)は、1999年にJenkinsonらによって開発された。開発当初、18名のALS患者への面接から78の候補項目が抽出された。その後、質問紙調査を経て、これらの項目は5つの健康状態領域を測定する40の質問項目に絞り込まれた。本尺度は、表面的妥当性、内的妥当性および構成概念妥当性が高いことが確認された。

2000年には、開発者らが500名の患者を対象に質問紙調査を実施し、折半法を用いて5領域の内的一貫性を確認することで、その信頼性および妥当性を確立した。2001年には、同じ質問紙が77名の患者に実施された。その後、疾患の進行に伴うスコアの変化が確認され、本尺度の臨床的妥当性が支持された。

Yamaguchiら16)は、標準化された異文化適応の手順を経て、40項目からなるALSAQ-40日本語版を開発し、日本人集団における妥当性および信頼性を確立した。

ALSAQ-40は、以下の5領域、計40項目で構成されており、各項目は5段階の回答尺度で評価される。

身体的移動(10項目): 立位の保持や歩行に関連する困難を評価する。歩行中の転倒経験や、階段の昇降が困難であったかなどが含まれる。

日常生活活動・自立(10項目): 物品の使用、更衣および食事に関する活動を評価する。ベッド上での寝返りや、更衣の困難などが含まれる。

食事・飲水(3項目): 食物や飲料を飲み込むことの困難など、嚥下に関連する問題を評価する。

コミュニケーション(7項目): 以前と比べて話す頻度が減少したかなど、円滑にコミュニケーションを行う能力についての本人の認識を評価する。

情緒機能(10項目): 孤独感や、個人の自由を完全に失ったという認識など、疾患に伴う否定的な感情体験を評価する。

各領域のスコアは0~100点の範囲に標準化され、得点が高いほどQOLが低いことを示す。ALSAQ-40の開発研究に用いられた標本は39名で構成され、そのうち64%が男性であった。平均年齢は58.5歳、平均罹病期間は39.1か月であった。参加者の44%が古典型ALSであり、46%が呼吸管理に関する介入を受けていた。したがって、本尺度は、診断後の平均罹病期間が3年を超える患者集団のデータに基づいて開発された。

Visual Analog Scaleを用いた主観的評価

各訓練セッションの終了後、参加者はVisual Analog Scale(VAS)を用いた8項目の主観的評価を実施した。評価項目は、満足度(付録4の項目1)、自己効力感(項目2および3)、ニーズ(項目4)、有用性の認識(項目5および8)、必要性の認識(項目6)および予後に対する不安(項目7)であった。得点が高いほど、より肯定的な回答であることを示した。

Amyotrophic Lateral Sclerosis Assessment Questionnaire-40 (ALSAQ-40)

The primary outcome was quality of life (QOL) in patients with ALS. The Amyotrophic Lateral Sclerosis Assessment Questionnaire-40 (ALSAQ-40), a disease-specific measure of QOL in patients with ALS, was developed by Jenkinson et al. in 1999. During its initial development, 78 candidate items were generated from interviews with 18 patients with ALS. Following a questionnaire survey, these items were reduced to 40 questions measuring five domains of health status. The instrument was found to have high levels of face, internal, and construct validity.

In 2000, the developers administered the questionnaire to 500 patients and used a split-half method to confirm the internal consistency of its five domains, thereby establishing its reliability and validity. In 2001, the same questionnaire was administered to 77 patients. Changes in scores associated with disease progression were subsequently confirmed, supporting the clinical validity of the instrument.15)

Yamaguchi et al.16) developed a 40-item Japanese version of the ALSAQ-40 through a standardized cross-cultural adaptation process and established its validity and reliability in a Japanese population.

The ALSAQ-40 comprises 40 items across the following five domains, each rated using a five-point response scale:

Physical Mobility (10 items): assesses difficulties related to maintaining a standing position and walking, including whether the patient has fallen while walking or experienced difficulty climbing or descending stairs.

Activities of Daily Living/Independence (10 items): assesses activities involving the use of objects, dressing, and eating, including difficulty turning over in bed or getting dressed.

Eating and Drinking (3 items): assesses swallowing-related difficulties, including difficulty swallowing food or drinking liquids.

Communication (7 items): assesses the perceived ability to communicate successfully, including whether the patient speaks less frequently than before.

Emotional Functioning (10 items): assesses negative emotional experiences associated with the disease, including feelings of loneliness or a perceived complete loss of personal freedom.

Scores for each domain are standardized to a range of 0-100, with higher scores indicating poorer QOL. The sample used in the ALSAQ-40 development study comprised 39 participants, of whom 64% were men. The mean age was 58.5 years, and the mean disease duration was 39.1 months. Classical ALS was reported in 44% of the participants, and 46% had received respiratory intervention. Thus, the instrument was developed using data from a sample with a mean disease duration of more than 3 years after diagnosis.

Subjective Evaluation Using a Visual Analog Scale

Following each training session, participants completed a subjective evaluation comprising eight items rated using a visual analog scale (VAS). These items assessed satisfaction (Item 1 in Appendix 4), perceived self-efficacy (Items 2 and 3), needs (Item 4), perceived usefulness (Items 5 and 8), perceived necessity (Item 6), and anxiety regarding prognosis (Item 7). Higher scores indicated more positive responses.
