本研究は、新型コロナワクチン接種後の健康被害の実態を明らかにし、その診断基準を作成し、そのような患者たちにより適切な医療を提供することを目的とする。そのために新型コロナワクチン接種後症候群の患者のレジストリを構築し、診断基準の作成のみならず、構築されたレジストリを基盤として、更に検査法の開発や適切な治療方法の開発に結び付けて、新型コロナワクチン接種による健康被害者を救済することに資する。

Our research seeks to create a registry of patients experiencing post-vaccination syndromes to meet this requirement. This initiative aims to illuminate the health effects post-COVID-19 vaccination, assist in formulating diagnostic criteria, and enhance patient care. It will pinpoint individuals with post-vaccination syndrome and those exhibiting symptoms associated with COVID-19 vaccination. Additionally, this study will concentrate on establishing diagnostic criteria and seek to impact the development of testing methodologies and suitable treatment approaches. Consequently, this research represents a critical step in assisting individuals impacted by health issues following vaccination, markedly contributing to the continuous battle against the COVID-19 pandemic. Our endeavors guarantee that vaccine deployment and usage are underpinned by extensive, transparent safety data, offering crucial insights for refining vaccine strategies and confirming their effectiveness.

・本レジストリへの登録に文書による同意が得られた患者
・各研究機関に2020年12月以降2023年8月31日までに通院又は入院した患者で、新型コロナワクチンとの関連性が確実もしくはほぼ確実と研究者に判断された症状を有する患者

有害事象は計493件。69.4％が接種後90日以内に発症、12.4％は360日以上経過後に発症。頻度の高い分類は「一般・全身障害」（29.2％）、「神経系障害」（22.3％）、「筋骨格系障害」（10.1％）であり、倦怠感、ブレインフォグ、関節痛、歩行障害、感覚鈍麻などが多く記録された。最大で29のSOC系統を呈した例も認められた。完全回復は27％、寛解38％、未回復29％、死亡1％。3SOC系統すべてに該当するPVSタイプ1では未回復率が63%と最も高かった。回復までの中央値は150～300日と長期化傾向を示した。

A total of 493 AEs were reported: 69.4% occurred within 90 days and 12.4% occurred >=360 days after vaccination. The most common SOCs were General Disorders (29.2%), Nervous System Disorders (22.3%), and Musculoskeletal Disorders (10.1%). Common symptoms were malaise, brain fog, arthralgia, gait disturbance, and hypoesthesia. Outcomes were complete recovery 27%, partial remission 38%, unresolved 29%, and death 1%. PVS Type 1 had the highest non-recovery rate (63%). Median recovery time was 150-300 days.

参加者背景/Baseline Characteristics

解析対象179例のうち、女性119例（66.5%）、男性60例（33.5%）であった。年齢は14～91歳で、同意取得時の平均年齢は59.3歳であった。COVID-19ワクチン接種回数は、1回17例（9.5%）、2回37例（20.7%）、3回76例（42.4%）、4回24例（13.4%）、5回以上25例（14.0%）であった。COVID-19感染歴を有する患者は47例（26%）、感染歴が報告されていない患者は131例（73%）であった。

Among the 179 patients included in the analysis, 119 (66.5%) were female and 60 (33.5%) were male. Ages ranged from 14 to 91 years, with a mean age at consent of 59.3 years. The number of COVID-19 vaccine doses received was one dose in 17 patients (9.5%), two doses in 37 (20.7%), three doses in 76 (42.4%), four doses in 24 (13.4%), and five or more doses in 25 (14.0%). Forty-seven patients (26%) had a history of confirmed COVID-19 infection, while 131 (73%) had no reported confirmed COVID-19 infection.
