目的1/Narrative objectives1

日本語
乳腺甲状腺外科外来リンパ浮腫看護外来に通院中で適切なセルフケアを行っているが浮腫改善がみられない難治性リンパ浮腫患者を対象に、ICGリンパ管蛍光造影検査を行いリンパ流の変化を確認する。ドレナージ可能なリンパ流を同定しその部分に向けたドレナージにセルフケア方法を変更することによるリンパ浮腫の変化を比較検討する。

For patients with refractory lymphedema who are attending the breast and thyroid surgery outpatient lymphedema nursing outpatient clinic and who are performing appropriate self-care but whose edema has not improved, ICG lymphangiography is performed to confirm changes in lymph flow. . To compare and examine the changes in lymphedema caused by identifying drainable lymph flow and changing self-care methods to direct drainage to that area.

患側上肢の浮腫状況を、上肢の周径採寸及びインボディ測定による上肢の細胞外液　
量の変化により確認する。

The edema status of the affected upper limb can be determined by measuring the circumference of the upper limb and in-body measurement of extracellular fluid in the upper limb.
Confirm by change in amount.

選択基準/Key inclusion criteria

日本語
対象者の選択基準は、以下の条件全てを満たしたものとする。
・乳癌術後で、続発性リンパ浮腫と診断された患者
・リンパ浮腫看護外来に通院中であり、セルフケアを開始していること
・複合的治療を行っており、スリーブによる圧迫療法を導入していること
・上記治療を行っているが、浮腫の改善を認めない難治性リンパ浮腫を抱える患者
・20歳以上であること

除外基準/Key exclusion criteria

日本語
除外基準は、以下のいずれかの条件に該当するものとする。
・腋窩を含む所属リンパ節の再発がある
・現在、精神疾患もしくはがん以外の重大なコントロール不良の身体疾患がある
・ヨードアレルギーがある（ICGは成分にヨウ素を含む）
・その他、研究参加に不適切であると主治医が判断した

主な結果/Results

日本語
対象者は10名（平均年齢±SD=58.9±9.0歳）、脱落者はいなかった。リンパ流は8名で同定され、うち4名は前腕のみの同定であり上腕では同定されなかった。リンパ浮腫の変化については、周径変化があり浮腫が改善した患者は8名、患側上肢の水分量が減少した患者は7名、皮膚の硬さや重だるさなど体感が改善した患者は6名であった。
また、リンパ流が同定できない患者でも、ドレナージの手技を再指導し生活指導も合わせて行ったことで、周径や水分量が減るなど介入効果があった。

Ten subjects participated, with no dropouts. Lymphatic flow was identified in eight patients, of which four were identified only in the forearm and not in the upper arm. Regarding changes in lymphedema, eight patients experienced a change in circumference and improvement in edema, seven patients experienced a decrease in fluid volume in the affected upper limb, and six patients experienced improvements in physical sensations such as skin hardness and heaviness.
