目的1/Narrative objectives1

本研究では4D Flow MRIを用いて術前，及び術後1週間，術後1年で大動脈弁から弓部大動脈における血流動態の変化を外科生体弁群，外科機械弁群，経カテーテル的大動脈弁留置術low flow low gradient AS生体弁群に対して解析し，比較検討を行う．また，low flow low gradient severe aortic stenosisや大動脈弁置換術後弁機能不全を疑う患者に対しても4D Flow MRIを用いて解析を行い，これらの病態に特徴的なパラメーターがあるのか検証する．

In this study, we will use 4D Flow MRI to analyze and compare changes in blood flow dynamics in the aortic arch from the aortic valve before surgery, one week after surgery, and one year after surgery in the following groups: surgical bioprosthetic valve group, surgical mechanical valve group, and transcatheter aortic valve implantation low flow low gradient AS bioprosthetic valve group. We will also analyze patients with low flow low gradient severe aortic stenosis or suspected aortic valve dysfunction after aortic valve replacement using 4D Flow MRI to verify whether there are parameters that are characteristic of these conditions.

主要アウトカム評価項目/Primary outcomes

1次エンドポイント：stroke volume，forward flow，backwaｒd flow，regurgitant fraction，ejection fraction，pressure gradient及び，大動脈弁から弓部大動脈におけるstreamlines flow，helical flow，vortical flow，血流速度，energy loss，wall shear stress，flow displacement，及びこれらの項目の時間経過に伴う変化率に各群間で有意差の評価．

Primary endpoints: stroke volume, forward flow, backward flow, regurgitant fraction, ejection fraction, pressure gradient, streamlines flow from the aortic valve to the arch aorta, helical flow, vortical flow, blood velocity, energy loss, wall shear stress, and flow displacement. The primary endpoints: stroke volume, forward flow, backward flow, regurgitant fraction, ejection fraction, pressure gradient, and streamlines flow, helical flow, vortical flow, blood velocity, energy loss, wall shear stress, flow displacement, and the rate of change of these parameters with time in the aortic valve to the arch aorta.

①患者から研究参加の辞退申し出や同意の撤回があった場合
②登録後に適格性を満足しないことが判明した場合
③有害事象により研究の継続が困難な場合
④著しくコンプライアンス不良の場合
⑤研究全体が中止された場合
⑥その他の理由により、医師が研究を中止することが適当と判断した場合
