本研究では、茨城県内の7つの在宅医療を提供する医療機関の診療録データにより、パンデミック前後に新たに在宅医療を開始した患者像の変化を明らかにする。

This study will use medical record data from seven medical institutions providing home healthcare in Ibaraki Prefecture to identify changes in the profile of patients who newly initiated home healthcare before and after the pandemic.

2019年4月１日～2023年3月31日の4年間の間に、大和クリニック、セントラル総合クリニック、北茨城市民病院付属家庭医療センター、ひたち太田家庭医療診療所、笠間市立病院、神栖済生会病院、済生会土合クリニックを受診し、新規に在宅医療が開始となった患者

Patients who newly started home healthcare in seven clinics/hospitals in Ibaraki prefecture (Yamato Clinic, Central General Clinic, Kitaibaraki Municipal Hospital Affiliated Family Medicine Center, Hitachi-Ota Family Medicine Clinic, Kasama City Hospital, Kamisu Saiseikai Hospital, and Saiseikai Doai Clinic) during April 1, 2019 and March 31, 2023.

年齢、性別、居住場所（自宅、施設）、基礎疾患、家族構成、主介護者との関係、要介護度、利用している介護サービス、受診の経緯（自院の患者か他院からの紹介か）、在宅医療導入理由（主に在宅看取り目的かどうか）等
［訪問診療開始後の情報］
2023年9月30日までの往診回数、往診の主訴、入院の有無、入院の原因、COVID-19感染の有無や感染時の対応（在宅継続、入院）、在宅医療継続期間、転帰（入院、死亡（死因や死亡場所も含めて）、施設入所）等

The following information will be extracted from the medical records, and a descriptive analysis and split time-series analysis will be conducted on the changes before and after the COVID-19 pandemic.
<Basic information at the start of the home visit>
Age, gender, place of residence (home or facility), underlying disease, family structure, relation with the main caregiver, level of care required, care services used, history of consultation (whether the patient was referred from the hospital or not), the reason for introducing home healthcare (whether the main purpose is end-of-life care at home), etc.
<Information after the start of home medical care>
Number of house calls up to September 30, 2023, chief complaint of the house call, whether the patient was hospitalized or not, cause of hospitalization, presence or absence of COVID-19 infection, and place of care to infection (continued home care, hospitalization), duration of continued home care, outcome (hospitalization, death (including cause and place of death), admission to institution), etc.
