大腸憩室出血止血術の必要な患者を対象に、ピュアスタットを用いた止血術(単独療法またはEBL(Endoscopic Band Ligation)、Clipping併用療法)を行い、再出血率などの治療成績、偶発症に関するデータを収集し、その有効性と安全性を後ろ向きに検証すること

To perform hemostasis (monotherapy or combined therapy with EBL (Endoscopic Band Ligation) and Clipping) with PuraStat in patients with colonic diverticular bleeding in need of hemostasis, and to collect data on outcomes such as rebleeding rate and data on complications, and to retrospectively evaluate the efficacy and safety of this treatment.

大腸憩室出血患者25名にPuraStatが使用された。成功率はPuraStat使用群とPuraStat非使用群で同等であった（100% vs. 96.4%、P=1.000）。30日以内の再出血率は、PuraStat使用群の方がPuraStat非使用群よりも有意に低かった（4.0% vs. 20.9%、P=0.047）。多変量解析により、Purastatの追加投与が再出血リスクの低減と関連していることが明らかになった（オッズ比0.11、95%信頼区間0.01-0.95、P=0.045）。

PuraStat was used in 25 patients with colonic diverticular bleeding. The success rates were comparable between the PuraStat using group and the PuraStat non-using group (100% vs. 96.4%, P=1.000). PuraStat (3-D Matrix, Tokyo, Japan) is an absorbent localized hemostatic agent that utilizes self-assembling peptide technology. In this multicenter pilot study, we evaluated the efficacy and safety of endoscopic hemostasis using PuraStat in patients with colonic diverticular bleeding (CDB).
参加者の流れ/Participant flow

この研究では、最近の出血の徴候（SRH）を伴うCDBを発症した患者を対象とし、PuraStat単独療法、またはPuraStatと内視鏡的バンド結紮術（EBL）またはクリッピングを併用した併用療法による内視鏡的止血術を実施しました（グループA）。治療結果と有害事象が評価され、PuraStatを使用せずに内視鏡的止血処置を受けたCDBとSRHの既往歴のある患者（グループB）と比較されました。

This study involved patients who had CDB with stigmata of recent hemorrhage (SRH) and underwent endoscopic hemostasis with PuraStat monotherapy or combination therapy comprising PuraStat with endoscopic band ligation (EBL) or clipping (Group A). Treatment outcomes and adverse events were assessed and compared with those of a previous cohort who underwent endoscopic hemostasis without PuraStat for CDB with SRH (Group B).

The primary outcome was that the rate of rebleeding within 30 days was significantly lower in Group A than in Group B (4.0% vs. 20.9%, respectively; P=0.047). The mean time to rebleeding was 3 days in Group A and 2.7 days in Group B. No difference was found in the rates of perforation and 30-day mortality between the two groups.
