胸骨正中切開を伴う開心術における術後疼痛は心臓血管麻酔管理において大きな課題の一つである。新潟大学医歯学総合病院では、胸骨正中切開を伴う開心術における術後疼痛管理目的、ならびに、先取り鎮痛効果（Preemptive Analgesia）と期待して、全身麻酔導入時に胸横筋膜面ブロック(Transversus thoracic muscle plane block (TTPB))を行っている。内胸動脈をグラフトとして用いる冠動脈バイパス手術（Coronary artery bypass graft surgery (CABG)）において、仮にTTPBにより投与された局所麻酔薬が内胸動脈周囲の組織まで到達する場合、内胸動脈に何かしらの影響を与えるのではないかという懸念が残る。本研究の目的は、TTPBで投与された局所麻酔薬が内胸動脈領域まで到達するかについて検討することである。

Postoperative pain after open heart surgery with median sternotomy is one of the major issues in the cardiovascular anesthesia management. At Niigata University Medical and Dental Hospital, the transversus thoracic muscle plane block (TTPB) is performed during general anesthesia induction for the purpose of postoperative pain management and preemptive analgesia effect. In coronary artery bypass graft surgery (CABG) using the internal thoracic artery as a graft, there is concern that it may affect the internal thoracic artery, if the local anesthetic administered by TTPB reaches the tissue surrounding the internal thoracic artery region. The purpose of this study was to investigate whether local anesthetic administered by TTPB reaches the internal thoracic artery region.
