表在性非乳頭部十二指腸上皮性腫瘍(SNADET)に対する浸水下内視鏡切除(UEMR)では, 重力や腸管蠕動による切除標本の流出や水の逆流による嘔吐や誤嚥が時に問題となる. この問題を改善するため, 我々は右側臥位で行うUEMRを考案した. 右側臥位は左側臥位よりも十二指腸が低い位置となるため, 切除標本が肛門側へ流出するリスクが低下し, 注水量が減少するため, 嘔吐や誤嚥のリスクも低減すると考えた. そこで本研究ではSNADETに対して, 右側臥位によるUEMRの有用性を明らかにすることを目的とした.

During underwater endoscopic mucosal resection (UEMR) for superficial non-ampullary duodenal epithelial tumors (SNADET), vomiting and aspiration due to outflow of the resected specimen due to gravity and intestinal peristalsis and reflux of water are sometimes problems. To improve this problem, we devised UEMR to be performed in the right lateral decubitus position. In the right lateral decubitus position, the duodenum is in a lower position than in the left lateral decubitus position, which reduces the risk of the resected specimen spilling into the anus and reduces the amount of water injected. We thought that this would reduce the risk of vomiting and aspiration. Therefore, this study aimed to clarify the usefulness of UEMR in the right lateral decubitus position for SNADET.

上部内視鏡検査にて表在性非乳頭部十二指腸上皮性腫瘍と診断がされ、浸水下内視鏡切除の適応と判断された者（生検の有無は問わない）。
紙面で研究参加への同意が得られた者

Those who were diagnosed with superficial non-ampullary duodenal epithelial tumor by upper endoscopy and judged to be suitable for underwater endoscopic mucosal resection (regardless of whether or not a biopsy was performed).
Those who have given written consent to participate in the research.

除外基準/Key exclusion criteria

・胃または十二指腸に外科手術歴を有する者
・関節痛や身体障害のため右側臥位になれない者
・重度の腎機能障害を有する者
・高度に心機能が低下している者
・妊娠している者
・担当医師が不適当と判断した者
