健常人およびCOPD患者の6分間歩行試験中の下肢筋肉と前頭葉の酸素化状態を、近赤外分光法で測定し、健常人とCOPD患者、特に歩行時に低酸素血症となる患者の筋肉および脳の酸素化状態を比較する。

The oxygenation status of lower extremity muscles and frontal lobe during a 6-minute walk test in healthy subjects and COPD patients is measured using near-infrared spectroscopy to compare the oxygenation status of muscles and brain in healthy subjects and COPD patients, especially those who are hypoxic during walking.

主な結果/Results

日本語
健常群では、6分間歩行試験中に脳の酸素化状態に有意な変化は認められなかった。一方、COPD群では、Δoxy-HbおよびΔtotal-Hbの減少とΔdeoxy-Hbの増加が認められ、6分間歩行試験中に前頭前皮質において脱酸素化および虚血が生じていることが示唆された。

The healthy group showed no significant changes in cerebral oxygenation during the 6-min walk test. Meanwhile, the COPD group demonstrated a decrease in oxy-Hb and total-Hb and an increase in deoxy-Hb, suggesting desaturation and hypoperfusion in the frontal cortex during the 6MWT.

6名のCOPD患者（COPD群）と8名の健常人（60歳以上）

参加者の流れ/Participant flow

COPD患者は全員、東海大学病院に通院中の外来患者であり、研究開始前の少なくとも3か月間は臨床的に安定した状態であった。患者は6分間歩行試験（6MWT）中に低酸素血症（パルスオキシメータで測定した
血中酸素飽和度（SpO2）が88%以下）を呈したが、安静時のSpO2は93%以上であった。
健常な参加者は、心肺疾患や脳血管疾患を有さず、60歳以上の本学職員であり、ポスターを用いて募集された。

All patients with COPD were outpatients attending Tokai University Hospital and had clinically stable disease for at least 3 months before the study. Patients experienced hypoxemia (oximetry-measured
blood oxygen saturation (SpO2) 88% less or lower) during the 6MWT, but SpO2 at rest was 93% more or higher.
Healthy participants were staff members at our university aged >60 years who did not have cardiopulmonary or cerebrovascular disease and were recruited using posters.
