主な結果/Results

日本語
本研究により、理学療法士の病棟専従配置により、入院中のHCC患者のBIと身体機能は有意に改善することが明らかとなった。一方、入院中の転倒は全例、pre-frailもしくはfrailであり、転倒と身体機能の関連が示唆された。このように、病棟に理学療法士を専従配置は、身体機能の改善に有用である。

Full-time ward work of physical therapists improved physical function in patients with HCC, suggesting the usefulness of the maintenance and improvement system for ADL. Fall was a risk factor for worsening physical function. To prevent the worsening of physical function. The evaluation of falling risk by physical therapists may be important in hospitalized patients with HCC.

本研究では、2019年1月22日から2021年1月15日までに久留米大学病院消化器内科病棟に入院した患者で、組入基準と除外基準を満たした肝細胞癌患者242名を登録した。組入基準は、1) 20歳以上、2) HCCに対する治療を目的に入院した患者、3) 入院時にLFIを評価できた患者である。除外基準は、1) HCCに対する治療以外の目的で入院した患者である (n=50)。また、2)退院時にLFIを評価できなかった患者である (n=351)。

We enrolled 242 patients with hepatocellular carcinoma who were admitted to the gastroenterology ward of Kurume University Hospital between January 22, 2019, and January 15, 2021, and who met the inclusion and exclusion criteria were enrolled. Inclusion criteria were 1) over 20 years of age, 2) patients admitted for treatment for HCC, and 3) patients who were able to assess LFI at admission. Exclusion criteria were: 1) patients hospitalized for non-therapeutic purposes for HCC (n=50); and 2) patients for whom LFI could not be evaluated at discharge (n=351).

病棟に理学療法士が専従勤務した。患者の日常生活動作能力はBarthel Indexによって評価され、身体機能は、握力、5回椅子立ち上がりテスト、バランステストによって評価された。

A physical therapist worked full-time in the ward. Patients' activities of daily living were assessed by the Barthel Index, and physical function was assessed by grip strength, chair stand test-5, and balance test. Patients were treated to prevent falls and decline of physical functions as needed.

身体機能低下は癌患者の予後不良とかかわるため、それらの改善は重要である。本研究では、リハビリテーション専門職の病棟専従配置による転倒と日常生活動作能力低下率改善効果に加えて、身体機能の変化も検討する。
