「慢性病をもつ人のセルフケア能力を高める看護支援プログラム（本庄,2015）」の手順を参考に介入する。セルフケアにおける強みと弱みを捉え、対話を通して具体的なセルフケアの方法を看護師と患者でともに考えるICTを活用した支援を行う。

The intervention will be based on the procedures of "Nursing Support Program for Enhancing Self-Care Skills of People with Chronic Illness (Honjo,2015)". The Support is provided using ICT to assess strengths and weaknesses in self-care and to think together with patients about specific self-care strategies through dialogue.

PCI治療後6ヶ月以上が経過している循環器内科外来に通院する患者で、かつICT による通信機器を持っており、機器の操作と通話が可能な者

主な結果/Results

日本語
対象者は50代から80代の男性4名、女性2名であり、ICT活用状況はビデオ通話4名、電話通話2名であった。SCAQ-30の評価に基づき対象者とともに課題と目標を設定し、強みを評価しながら自己主導型学習とパートナーシップによる支援を行った。支援後のSCAQ得点は1から9点の上昇を認め、支援を行った下位尺度項目の変化を認めた。血液データや症状の悪化は認めなかった。プログラムの有用性の質問紙には、支援の頻度とICTの活用の適切性、タイムリーな症状への対処や相談の機会が得られたことの記述があった。

The subjects were 4 males and 2 females and ICT utilization was 4 video calls and 2 telephone calls. Based on the SCAQ-30 assessment, tasks and goals were set with the subjects, and support was provided through self-directed learning and partnership while assessing strengths. After the intervension, SCAQ scores increased by 1 to 9 points. No worsening of blood data or symptoms was observed.
