研究デザイン：前向き観察研究。対象：現場で救急業務を実施している救急救命士を対象にアナフィラキシーの判断能力を評価する。調査項目：救急現場において救急救命士によってなされたアナフィラキシー・アドレナリン投与適応の判断結果を、救急搬送後の医療機関において医師によってなされた判断結果と比較し、陽性的中率、陽性尤度比等を評価する。
対象者の選定方針（救急救命士）：全国の都道府県・地域メディカルコントロール（MC）協議会、消防本部などの団体に対する公募を行う。参加団体に所属する救急救命士に対し、説明文書・動画等で説明のうえ同意を得る。参加団体の研究参加管理担当者において同意の確認とともに、対象者（救急救命士）に対して研究班が準備する事前学習e-ラーニング（内容：本研究の概要、アナフィラキシーの病態等の基礎知識、本研究におけるアナフィラキシー・アドレナリン投与適応の判断、観察カードの使用）を案内する。事前学習を修了した救急救命士においてデータ収集を行う。

Research design: a prospective observational study. Aim and Methods: to evaluate the ability of emergency life-saving technicians (ELSTs) to diagnose anaphylaxis by comparing the diagnoses of anaphylaxis requiring adrenaline administration by ELSTs in the field with those made by physicians at medical institutions after transport. The study will evaluate the positive predictive value and positive likelihood ratio and others of the decisions by ELSTs against the decisions by the physicians. The selection of ELSTs will be done through public recruitment to organizations such as prefectural and regional medical control (MC) committees and fire departments. ELSTs who belong to participating organizations will be provided with explanatory documents and videos and asked to provide consent. The study team will guide ELSTs who have completed the preparatory e-learning (which includes the overview of the study, basic knowledge of anaphylaxis, judgments of anaphylaxis and adrenaline administration in this study, and the use of observation cards) to collect data.
