CSPを用いて切除したSSL標本の垂直断端について病理組織学的に検討した研究は少ない。さらに、SSLに対するCSPの妥当性についても、まだ決定されていない。そこで、本後ろ向き研究では、EMRと比較して、CSPで治療したSSLにおける垂直・水平断端の陽性および評価不能割合、粘膜筋板の切除割合を明らかにすることを目的とした。有意差を認めれば、その後の長期再発の比較検討を行う。

There is a lack of studies that have histopathologically examined the vertical margins of SSL specimens resected using CSP. Furthermore, the appropriateness of CSP for SSLs has yet to be determined. This is a prospective cohort study comparing the local recurrence rates of CSP and EMR for SSL. Since recurrence is caused by the residual tumor in resected area, we evaluate the incidence of positive vertical margins in histopathological examination in this interim analysis. If there is no significant difference in the rate of positive vertical margins, we determined that long-term observation was not warranted and this cohort study would be terminated.

CSPかEMRで切除したSSL症例

Consecutive patients treated with CSP or EMR and histopathologically diagnosed with SSL at the Southern-Tohoku General Hospital in Fukushima, Japan, between January 2019 and December 2022 were included in this study. SSLs less than 10 mm in diameter by endoscopic measurement and achieved en-block resection were included in this study.

除外基準/Key exclusion criteria

炎症性腸疾患患者、分割切除、進行癌合併、遺残病変

Exclusion criteria were as follows: cases with more than 6 resections; underwent piecemeal resection; advanced cancer coexistence; serious comorbidities; history of inflammatory bowel disease; difficulty obtaining consent due to psychiatric disorder.

我々は、SSLに対するCSPとEMRの垂直断端陽性の発生率と長期成績を比較するコホート研究を行った。現在、切除後の再発率に関する追跡研究を計画中である。本論文では、このコホートにおける切除標本の組織学的評価について報告する。前向き観察研究。
