The Community Health Center Alianyang（介入群） または Community Health Center Sei Durian（対照群）を受診した18歳以上の糖尿病患者

Patients with diabetes go to the Community Health Center Alianyang (Intervention Group) or Community Health Center Sei Durian (Control Group) and over the age of 18.

除外基準/Key exclusion criteria

日本語
足の大切断の既往のある患者、12か月間この研究に参加できない患者、インドネシア語が話せない患者、全身状態が研究に適さない患者（主治医の判断）

英語
Patients with large lower limb amputation, cannot participate in this study for 12 months, do not speak Indonesian, medical condition not suitable for the study (judge by physician-in-charge)
