ロピバカインとフェンタニルを用いた無痛分娩から帝王切開に移行した際の母体および臍帯血の麻酔薬濃度を測定し、その濃度と出生した新生児の健康状態について検討する。

We measure maternal and umbilical blood anesthetic concentration in the case of conversion from labor analgesia to cesarean section using ropivacain and fentanyl. We study the relationship between the anesthetic concentration and neontal health status.

日本語
出生直後の新生児の健康状態について評価する。新生児の健康状態としてはアプガースコア・臍帯動脈血pH・新生児蘇生・NICU/GCUへの入室を含める。

英語
We asses the neonatal health status immediately after birth. The neonatal health status includes Apgar score, umbilical cord artery blood pH, neonatal resuscitation, and admission to the NICU/GCU.

副次アウトカム評価項目/Key secondary outcomes

帝王切開の麻酔薬投与開始から帝王切開終了までの母体の状態を評価する。母体の状態として局所麻酔中毒・呼吸抑制の有無を評価する。

We evaluate the condition of the mother from the beginning of anesthetic administration for cesarean section to the end of cesarean section. We evaluate the presence of local anesthetic systemic toxicity and respiratory depression as a maternal condition.

日本語でのコミュニケーションが可能であること
単胎の妊娠であること
分娩時期が37週以上42週未満

Pregnant women plannning to undergo labor analgesia.
Pregnant women who can comunicate in Japanese.
Pregnant women with singlton pregnancy.
Pregnant women shose delivery in between 37 and 42 weeks gestation.

帝王切開用の硬膜外麻酔薬投与前の母体静脈血と分娩時の母体静脈血及び臍帯静脈血を採取し、臍帯血麻酔薬濃度を測定する。
測定した麻酔薬濃度と出生した新生児の健康状態について検討する。新生児の健康状態はアプガースコア・臍帯動脈血pH・新生児蘇生、NICU/GCUへの入室を含む。
また、測定した麻酔薬濃度と母体の状態について検討する。母体の状態は局所麻酔薬中毒の有無・呼吸抑制の有無を含む。

英語
We measure maternal venous anesthetic concentraion before drug administration and at delivery. We also measure umbilical venous anesthetic concentration at delivery.
We study the relationship between the anesthetic concentration and neonatal health status. The neonatal health status includes Apgar score, umbilical cord artery blood pH, neonatal resuscitation, and NICU/GCU admission.
We study the relationship between the anesthetic concentration and maternal health status.
Maternal status includes the presence or absence of local anesthetic systemic toxicity and respiratory depression.
