本研究の目的は、足部・足関節手術を受ける患者に対し、腰椎麻酔または全身麻酔後に足関節を牽引し、牽引前後の浅腓骨神経と深腓骨神経の動態を超音波を用いて評価することである。
本研究により、これまで検証されていなかった足関節牽引による神経の動態を知ることができ、その移動量を超音波を用いて定量化することが出来る。本研究の結果は、足関節鏡による医原性神経損傷を回避することができ、ひいては治療成績の向上に寄与する。

The purpose of this study is to evaluate the dynamics of the superficial and deep fibular nerves before and after traction in patients undergoing foot/ankle surgery after spinal or general anesthesia using ultrasound. That is.
Through this study, it is possible to know the neural dynamics due to ankle joint traction, which has not been verified so far, and to quantify the amount of movement using ultrasound. The results of this study can avoid iatrogenic nerve damage caused by ankle arthroscopy, and thus contribute to the improvement of treatment outcomes.

2023年3月以降に千葉大学医学部附属病院足の専門外来を受診し、足部・足関節手術を受ける患者を対象とする。手術当日の手術前に超音波診断装置を用いて浅/深腓骨神経の皮膚からの距離を計測する。また、浅腓骨神経は第3腓骨筋腱外側縁からの距離を、深腓骨神経は長母趾伸筋腱外側縁からの距離も計測する。麻酔後に再度同じように計測し、麻酔前後でのそれぞれの変化量を算出し神経移動量を求める。

Patients undergoing foot/ankle joint surgery after March 2023 at Chiba University Hospital are eligible. On the day of surgery, the distance from the skin to the superficial/deep fibular nerve is measured using ultrasonography before surgery. Also measure the distance from the lateral edge of the third peroneal tendon for the superficial peroneal nerve, and the distance from the lateral edge of the extensor hallucis longus tendon for the deep peroneal nerve. After anesthesia, the same measurements are taken again, and the amount of change before and after anesthesia is calculated to determine the amount of nerve migration.

手術当日の1日だけ行う。麻酔後に足関節にストラップを装着し牽引装置に繋げて足関節を牽引する。牽引力は5kgf程度で牽引時間は5分程度である。

Only performed on the day of surgery. After anesthesia, a strap is attached to the ankle joint and connected to a traction device to pull the ankle joint. The traction force is about 5 kgf and the traction time is about 5 minutes.

2023年3月以降に千葉大学医学部附属病院足の専門外来を受診し、足部・足関節手術を受ける患者。

除外基準/Key exclusion criteria

日本語
１）足関節に手術の既往がある、２）足関節に外傷性による傷がある、３）Body mass index 30以上の肥満がある、４）腰椎麻酔下で長時間手術が予定されている、５）18歳未満

1) History of ankle surgery, 2) Traumatic injury to the ankle joint, 3) Obesity with a Body mass index of 30 or higher, 4) Long-term surgery is scheduled under lumbar spinal anesthesia, 5) Under the age of 18
