わが国は、人口の約28%が65歳以上の高齢者である世界一の超高齢社会である。高齢化にともない寝たきりや要介護などの日常生活に制限のある期間が長期化しており、患者自身のQOL（Quality of Life）低下、介護士不足や医療費の増加が問題となっている。健康寿命を延伸していく対策の鍵として、厚生労働省はフレイル・サルコペニアの予防を掲げている。さらに2020年からは、世界規模の新型コロナウイルス感染症の長期化が、高齢者の日常生活動を制限して、運動制限を強いることになりフレイル・サルコペニアの進展を惹起している。わが国の地域在住高齢者におけるフレイル・サルコペニアの有病率の報告はあるが、複数疾患を抱えることの多い外来通院患者におけるフレイル・サルコペニアの有病率は不明である。

Japan is the world's largest super-aging society, with approximately 28% of the population aged 65 or older. The aging of the population has resulted in a prolonged period of bedridden patients and those requiring nursing care and other restrictions on daily living, which has led to a decline in the quality of life (QOL) of the patients themselves, a shortage of careworker, and an increase in medical costs. The Ministry of Health, Labour and Welfare (MHLW) has identified prevention of frailty and sarcopenia as a key measure to extend healthy life expectancy. Furthermore, from 2020, the prolonged global outbreak of COVID-19 infection limited the daily activities of the elderly, forcing them to restrict their exercise, and thus triggering the development of frailty and sarcopenia. Although the prevalence of frailty and sarcopenia among community-dwelling elderly in Japan has been reported, the prevalence of frailty and sarcopenia in outpatients, who often suffer from multiple diseases, is unknown.

当院では65歳以上で当医療センター通院患者を対象にフレイル・サルコペニアの観察コホート研究を2021年から行なっているが、今回は、内科系以外の診療科に通院する患者も対象にフレイル・サルコペニアの有病率および関連因子に関する横断研究を計画した。

We have been conducting an observational cohort study of frailty and sarcopenia in patients aged 65 years or older attending our medical center since 2021, and this time we planned a cross-sectional study on the prevalence of frailty and sarcopenia and related factors in patients attending departments other than internal medicine.

3 自立歩行 (杖歩行含む)
4 研究参加の同意得られた患者

1 Regular outpatient at Juntendo Tokyo Koto Medical Center for the Elderly, Juntendo University School of Medicine
2 Age 65 or older
3 Independent walking (including walking with a cane)
4 Patients with consent to participate in the study

除外基準/Key exclusion criteria

日本語
1 自立歩行不可または問診・質問票聴取不可の患者
2 重篤な脳血管疾患、心疾患、呼吸器疾患、肝疾患、腎疾患、消化器疾患、内分泌代謝疾患患者
3 悪性疾患などにより余命が1年以内と判断された患者
4 主治医が不適格と判断した患者

1 Patients who are unable to walk independently or who are unable to take a medical history/questionnaire
2 Patients with serious cerebrovascular, cardiac, respiratory, hepatic, renal, gastrointestinal, or endocrine-metabolic diseases
3 Patients whose life expectancy is judged to be within 1 year due to malignant diseases
4 Patients deemed ineligible by the attending physician
