通常の内科診療に合わせて移行プログラムを実施する。プログラムは小児科から成人診療科への移行医療に関する内容を含み、4回程度実施する。多専門職種からなるチームメンバーが当院外来または病棟で実施する。

The transition program will be conducted in conjunction with regular clinical practice. The program includes content related to health care transition from pediatrics to adult practice and is conducted approximately four times. A multispecialty team member will provide education session or consultation in the outpatient clinic or in the hospital ward.

選択基準/Key inclusion criteria

日本語
成人診療科での診療や福祉サービス利用を目的とし、当院内科を初診する40歳未満の小児期発症神経系疾患患者とその家族で、移行プログラムへの参加を希望し、研究参加同意の得られた患者・家族を対象とする。

Patients with childhood-onset neurological conditions under 40 years of age and their families who make a first visit to our department seeking for treatment in an adult department or to use social services, who wish to participate in the transition program and have given consent to participate in the study will be eligible.

除外基準/Key exclusion criteria

緊急性の高い医療・社会的問題（緊急に医療的処置が必要、虐待の疑いがある場合など）がある患者・家族は対象外とする。

Patients/families with urgent medical or social problems (i.e., urgent need for medical intervention, or suspected abuse) are not eligible in this study.
