1) 初回時に、通院中の統合失調症患者
2) 20歳以上かつ60歳未満の患者
3) 被保険者（本人）として職場の健康保険に加入している患者
（※健康保険の対象となっている従業員や公務員などの本人のことであり、その家族（被扶養者）や、国民健康保険加入者ではない。
4) 評価時にWebを介してアンケート調査の実施が可能な患者
5) 本人の同意が得られた患者

2) Patients aged 20 and over, and less than 60 years
3) Patients who have joined the health insurance
4) Patients who are able to respond in the web-basis
5) Patients who agree with the ICF
