本研究の目的はテント上脳腫瘍患者における手術前後の運動転帰と、Diffusion Tensor Imaging（DTI）及びregional Cerebral Blood Flow(rCBF) との関係を検討することである。

脳腫瘍患者の運動機能を入院時と退院時にManual Muscle Test（MMT）を用いて評価した。術前の運動麻痺有群と無群において、DTI上の皮質脊髄路(Corticospinal tract : CST)(大脳脚、内包後脚、放線冠）におけるDTI指標を比較した。また、両群においてCSTにおけるrCBFは、病側/健側と病側/同側小脳半球で評価した。
次いで、術前運動麻痺有群のなかで術後麻痺改善した群と不変、悪化群において、DTI上のCSTのDTI指標、rCBFを検討した。

We evaluated motor function in patients with brain tumors using the Manual Muscle Test (MMT) on admission and at discharge. DTI parameters in the corticospinal tract (CST) (cerebral peduncle, posterior limb of internal capsule, and corona radiata) on the DTI were compared between patients with and without preoperative motor paralysis. The rCBF in the CST on the DTI in both groups was also evaluated in the diseased/healthy and diseased/ipsilateral cerebellar hemispheres.
We examined the DTI index of CST on DTI and rCBF in the group with improved postoperative paralysis and the group with unchanged and worsened postoperative paralysis.

2016年1月から2022年6月までの期間に東邦大学医療センター大森病院で外科的治療を施行したテント上脳腫瘍患者の中で、手術前にDTIとSPECTが撮影可能であった84例を対象とした。

本研究は東邦大学大森病院倫理委員会の承認を得て後方視的症例対照研究として実施した。2016年1月から2022年6月までの期間に東邦大学医療センター大森病院で外科的治療を施行したテント上脳腫瘍患者の中で、手術前にDTIとSPECTが撮影可能であった84例を対象とした。診療録から抽出したデータを解析し、診療録に提出された情報（病歴、手術前後の麻痺所見、画像所見）を、個人情報が分からないように管理した。

This study was conducted as a retrospective case-control study after obtaining approval from the Ethics Committee of Toho University Omori Hospital. 84 patients with supratentorial brain tumors who underwent surgical treatment at Toho University Medical Center Omori Hospital between January 2016 and June 2022, of whom DTI and SPECT were taken before surgery, were included. We analyzed the data extracted from the medical records and managed the information in the medical records (medical history, findings of paralysis before and after surgery, and imaging findings) in a way that personal information was not revealed.
