本研究では日本の実臨床におけるDOAC内服患者の背景因子、DOACの種類、DOACの内服用量を含めた背景データと患者の予後データを用いて、より安全かつ効果的なDOACの使用につながるよう、さまざまな見地から多角的に解析する。増加の一途を辿るDOAC処方患者の現状を理解するため、様々な施設形態における日本の実臨床を反映する大規模なリアルワールドデータを統合し、より一般化可能性の高い日本人に適したエビデンスの創出を目指す。様々な側面から多角的かつ探索的に研究を行い、このレジストリでDOACの適正な使用方法の新たな体系を構築する。

Number of AF (atrial fibrillation) patients treated with direct oral anticoagulants is becoming larger. This study will comprehensively analyze the large database which consists of three large-scale registries, and establish the real-world evidences.

日本語
DIRECT registry、SAKURA-AF registry、大阪大学医学部附属病院のデータを統合し、7,553例からなるDIRECT-Extend registryを構築した。異なる診療環境のリアルワールドデータを活用し、日本人におけるDOACの適正使用に資する一般化可能性の高いエビデンス創出を目指した。研究成果は国内外の学会で発表し、関連論文も複数報告した。

We established the DIRECT-Extend Registry by integrating the DIRECT Registry, SAKURA-AF Registry, and Osaka University Hospital database to generate generalizable real-world evidence on DOAC use in Japan. The registry includes 7,553 patients receiving anticoagulant therapy across diverse clinical settings. Multiple studies have been conducted using this database, with findings presented at EuroPCR 2024 and JCS 2025 and several related articles published.
