昭和大学藤が丘病院歯科・口腔外科では診療科固定の手術室使用枠がないにも関わらず近年症例数が大きく増大している。2016年4月から2022年3月までの6年間に昭和大学藤が丘病院で実施された静脈内鎮静法もしくは全身麻酔法で管理された歯科口腔外科手術症例を集計し、症例数増大の要因を検証した。

In recent years, the number of cases has increased greatly at SHOWA University Fujigaoka Hospital Dentistry and Oral Surgery, although they have no operating room. We collected dental and oral surgery cases treated with intravenous sedation or general anesthesia performed at SHOWA University Fujigaoka Hospital from April 2016 to March 2022. And we examined the factors behind the increase in the number of cases.

調査期間の前半の3年間と後半の3年間を比較すると，歯科麻酔科管理症例数は約2倍に増加した.歯科・口腔外科症例の95%は智歯抜歯であり，歯科診療の中でも比較的侵襲の大きな口腔外科手術では麻酔管理下での治療を希望する患者が一定の割合で存在していることが示された.この需要に対応するため，麻酔時間は静脈内鎮静では49±21分から35±9分(平均±標準偏差，前半vs後半)に短縮され(p<0.001) ，全身麻酔では1時間59分±38分から1時間33分±27分(p<0.001)に短縮していた.

A total of 465 cases were eligible for study inclusion, of which >95% of the patients underwent minor dental surgery. The time required for anesthesia significantly decreased from 49 <= 21 to 35 <= 9 min in intravenous sedation and from 119 38 to 93 +/- 27 min in general anesthesia (p <0.001). An experienced oral surgeon and a full-time dental anesthesiologist were appointed during this period. Patients who received intravenous sedation or general anesthesia were sustained at - 20% annually.

調査期間のうち症例数が大幅に増大した 2019年度から 2021年度に静脈内鎖静法もしくは全身麻酔法を用いた歯科麻酔科管理症例は462症例（患者数は415名、2回治療を受けた患者は45名、3回治療を受けた患者は1名）であり、平均年齢35土15歳、男性 180名（43%）。女性235名（57％）。合併患のないASA-PS 1が315名（75.9%）.軽度の全身疾患を有するが日常生活に問題がないASA-PS 2が97名
（23.4%）.高度の全身疾患を有する ASA-PS 3以上が3名（0.7%）であり、基礎疾患が併存する患者は112名（27.0%）であった（循環器疾患7.5%．呼吸器4.8%．神経0.5%．その他の疾患21.0%） ASA-PS 3以上の3名は透析患者1名、ペースメーカ挿入患者2名であった。

From FY2019 to 2021, There are 462 cases of dental anesthesiology management cases (415 patients, 45 patients received twice, 1 patient who received three treatments), average age of 35 +/- 15 years old, 180 men (43%), 235 women (57%), ASA-PS 1 315 (75.9%), ASA-PS 2 97 (23.4%), ASA-PS 3 3 (0.7%), and three were one dialysis patient and two pacemaker inserts.
