2) Patients who developed anaphylaxis and received an emergency visit or scheduled visit to the cooperating institution.

研究参加時と以降 3 年間の特定の時期(1 月、4 月、7 月、10 月)にweb上のアンケートで対象患者の下記情報を取得する。

・研究参加時
１ メールアドレス
２ 患者背景（年齢、性別、アナフィラキシー既往回数とその誘因、気管支喘息の合併と加療の有無）
３ アナフィラキシーの詳細(誘因、医師の診断の有無、運動誘発の有無)
４ アドレナリン自己注射製剤所持の有無

・２回目以降
１ 前回調査後からのアナフィラキシー発症の有無
２ アナフィラキシーの発症がある場合はその詳細(誘因、医師の診断の有無、運動誘発の有無)、アドレナリン自己注射製剤の使用の有無、救急搬送の有無、医療機関受診の有無

Research and Survey Items
<Methods>
The following information will be obtained from eligible patients via a web-based questionnaire at the time of study entry and at specific times (January, April, July, and October) during the following three years.

<Research Items>
First survey for participation in this study
1 E-mail address
2 Patient background (age, gender, number of previous anaphylaxis and its triggers, whether or not bronchial asthma is complicated and treated)
3 Details of anaphylaxis (triggers, physician's diagnosis or not, exercise-induced or not)
4 Possession of adrenaline self-injection products or not

The second survey and subsequent
1 Whether or not anaphylaxis has occurred since the last survey
2 If there has been an onset of anaphylaxis since the last survey, details of anaphylaxis (triggers, physician's diagnosis or not, exercise-induced or not), adrenaline self-injection products were used or not, emergency medical care was taken or not, a medical facility was visited or not.
