(1) 病前より日常生活動作に制限がある患者（病前modified Rankin Scale：mRS≧2）
(2) 精神状態短時間検査-日本語版(Mini Mental State Examination-J; MMSE-J) <= 23
(3) リハビリテーションからの脱落
