選択基準/Key inclusion criteria

(1) 臨床フレイルスケール４-６
(2) 外来に通院、もしくは入院中の心・血管疾患患者（安定狭心症、医学的に安定している心筋梗塞後、慢性心不全、心・血管手術術後）

(1)Clinical frailty scale 4-6
(2)Patients with cardiovascular disease (stable angina, medically stable post-myocardial infarction, chronic heart failure, post-operative cardiac or vascular surgery) who are in outpatient clinics or in hospital
除外基準/Key exclusion criteria
日本語
(1)不安定狭心症、コントロールされていない高血圧（収縮期血圧180/mmHg以上もしくは、拡張期血圧110/mmHg以上）、重度の大動脈弁狭窄症、コントロールされていない不整脈、非代償性心不全、急性大動脈解離
(2)研究責任者が不適当と判断した患者

(1)Unstable angina, uncontrolled hypertension (systolic blood pressure > 180/mmHg or diastolic blood pressure > 110/mmHg), severe aortic stenosis, uncontrolled arrhythmia, decompensated heart failure, acute aortic dissection
(2)Patients who judged to be not appropriate by the principal investigator.
