つくば市が行政利用のために実施した生活習慣および糖尿病に関するアンケート調査結果を元に，糖尿病患者の療養の課題を抽出する．またつくば市が保有する，アンケート調査とつくば市の特定健康診査・レセプト情報が突合されたデータを二次利用し，患者背景の特性を解析する．

Based on the results of a questionnaire survey on lifestyle and diabetes conducted by Tsukuba City for administrative use, we will extract issues related to the medical treatment of patients with diabetes mellitus. We will also analyze the characteristics of patient backgrounds by using the data from Tsukuba City, which is a combination of the questionnaire survey and the information of specific health checkups and claims data of Tsukuba City.

2022年9月時点でつくば市の国民健康保険に加入しており，つくば市在住で2022年10月31日時点で20～74歳の集団から糖尿病患者を選択した：
①　2021年度に糖尿病薬処方（ATC分類：A10）を1回でも受けている者
②　2021年度健診でHbA1c 6.5%以上であり，同年度に糖尿病薬処方を受けていない者
この糖尿病患者集団から，層別無作為抽出法で抽出された1,000人の患者に調査票を郵送し、有効回答を以て研究対象者に組み入れた。

Patients with diabetes were selected from the population who were enrolled in the National Health Insurance of Tsukuba City as of September 2022, residing in Tsukuba City and aged 20-74 years as of October 31, 2022:
(1) Those who received at least one diabetic drug prescription (ATC classification: A10) in FY2021.
(ii) Those who had HbA1c of 6.5% or higher in the medical checkup in FY2021 and had not received a prescription for diabetes medication in the same year.
From this population, questionnaires were mailed to 1,000 patients selected by stratified random sampling method, and valid responses were used to include them in the study.

除外基準/Key exclusion criteria

2021年度に妊娠糖尿病の病名（O244、O249）があったものを除いた．
サンプリング時（2022年11月）に被保険者でなくなった患者，糖尿病を自覚していない患者は研究対象から除外した．
さらに，研究に関連する設問に対して無回答あるいは複数回答の場合には無効回答として除外した．

Patients with gestational diabetes mellitus (O244 and O249) in FY2021 were excluded.
Patients who were no longer insured at the time of sampling (November 2022) and patients who were unaware of their diabetes were excluded from the study.
In addition, no response or multiple responses to questions relevant to the study were excluded as invalid responses.

医療者から眼科受診勧奨をうけた認識を持つものは47.6%, 眼科受診頻度の知識を持つものは72.8％、眼底検査を実施した人は50．5％であった．眼底検査の実施割合は、眼科受診勧奨の認識がある群72.9％ 対 眼科受診勧奨の認識がない群30.1％(p<0.001)、眼科受診頻度の正しい知識をもつ群63.9％ 対 眼科受診頻度の正しい知識をもたない群21.1％(p<0.001)であった。眼科受診頻度の知識をもつ割合は、眼科受診勧奨の認識がある群93.4％ vs 眼科受診勧奨の認識がない群49.6％(p<0.001)であった。いずれも有意な関連が示唆された。

アンケート調査の対象者は2022年9月時点でつくば市の国民健康保険に加入しており，つくば市在住で2022年10月31日時点で20～74歳の個人のうち，以下の2つの条件に従って糖尿病を持つと定義された患者から選んだ：
1)　2021年度に糖尿病薬処方（ATC分類：A10）を1回でも受けている者
2)　2021年度健診でHbA1c 6.5%以上であり，同年度に糖尿病薬処方を受けていない者
抽出された糖尿病患者集団（n = 3,450）から，層別無作為抽出法で抽出された1,000人の患者に調査票を郵送した。

The patients with diabetes for the questionnaire survey were extracted from insurance claims data and health check-up data. The first inclusion criterion identified individuals covered by Tsukuba City's National Health Insurance as of September 2022 and aged 20-74 years as of October 31, 2022. From this group, patients with diabetes were selected according to the following conditions: (1) Those who have received at least one antidiabetic drug prescription (ATC classification : A10) in FY2021. (2) Those whose HbA1c was 6.5% or more in the health check-up in FY2021 and who have not received any diabetic medication prescription in the same year.
From the selected source population of patients with diabetes (n = 3,450), questionnaires were mailed to 1,000 patients chosen by a stratified random sampling method.

主要アウトカムは，実際に眼底検査を受けた患者の割合とした．副次アウトカムは，望ましい眼科受診頻度に関する正しい知識を持つ者の割合とした．望ましい眼科受診頻度は，日本における糖尿病診療ガイドライン（2019年版）に基づき「半年あるいは1年に1回以上」とした．

The primary outcome was actual participation in the fundus examination.
The secondary outcome was knowledge of the recommended frequency of eye checks.
Having knowledge of the recommended frequency of eye examinations was defined as selecting 'once every 6 months or more' or 'once a year.' based on the clinical guidelines in Japan.

本研究では，つくば市が国民健康保険計画策定への行政利用を目的に実施した一般被保険者および糖尿病患者向けアンケート調査結果を二次利用する．アンケート結果をつくば市の特定健康診査結果・レセプトデータ・国民健康保険課が有する保険者台帳等と突合する．背景情報や回収率を勘案して重みづけを行い，単純集計，属性クロス集計および自由記述の整理を行う．

In this study, we use the results of a questionnaire survey conducted by Tsukuba City for patients with diabetes and the general population for the purposeof using the results in the formulation of the National Health Insurance Plan.
The results of the questionnaire will be matched with the results ofspecific health checkups, claims data, and the insurer registry held by the National Health Insurance Department of Tsukuba City. We will make theweights considering the background information and the response rate, and organize the simple tabulation, attribute cross tabulation, and freedescription.
