新型コロナの終息は未だに見えず、現在医療関係者を中心に4回目のワクチン接種が施
行されている。また、ファイザー社はオミクロン株に対するワクチン開発も完了したと
の報告があり、将来5回目のワクチン接種の可能性もある。これほどのワクチン筋注が
全世界で施行されたことは人類歴史上なく、延べ人数が増えるに従って合併症の報告も
散見されるようになった。典型的なものとして腋窩神経や後上腕回旋動脈の損傷、三角
筋下滑液包への誤注入などがあり、これら合併症は総じてShoulder Injury Related to
Vaccine Administration (SIRVA)と呼ばれ、本邦でも医療訴訟に発展したケースがあ
る。
従来の筋注刺入点として肩峰から3横指下の三角筋内が適切と言われていたが、あくま
で今までの経験則からであり、画像を使ったエビデンスからの情報ではない。本研究の
目的はボランティアを用いて、三角筋周辺にエコープローブを当て、従来の筋注刺入点
からの神経、動脈、関節包などの距離を測定し、より合併症の少ない新たな筋注刺入点
をエコー画像上見出すことである。

This is the first time in human history that such so many intramuscular injections of vaccines by COVID 19 has been carried out worldwide. In proportion of the total number of vaccine receivers, there has been increased reports of complications, such as damage to the axillary nerve, posterior brachial circumflex artery, by triangular intramuscular injections.
These complications are generally called as shoulder injury related to vaccine administration (SIRVA), that have developed into medical lawsuits.
The purpose was to research volunteers placing an echo probe around the deltoid muscle, and we measure the distance from nerves, arteries, joint capsules, etc., and create a new muscle injection point with less complications using echo image.

主要アウトカム評価項目/Primary outcomes

ボランティアに対し、三角筋にエコーを当て腋窩,橈骨神経走行を連続線で描出する。
ワクチン刺入点も表記し、静止画として保存して解析を行う。その際、腕のポジション
（内旋・外旋）を一定に保つ。各神経同定を確実にするために神経刺激装置を用いる場
合もある。
