目的/Objectives

日本語
加圧抜管と平圧抜管後の術後肺炎のリスクを比較することが目的である。筑波大学附属病院麻酔科では、気管内挿管を伴う全身麻酔後の抜管は加圧抜管でおこなっていた。しかし、コロナウイルス感染症の流行後は、抜管時の飛沫感染を予防するために、抜管方法を加圧抜管から平圧抜管に切り替えた。加圧抜管は理論上、誤嚥性肺炎の予防に有効とされるが、その有効性を確認したヒトの研究はない。そこで本研究では、加圧抜管と平圧抜管の術後肺炎の発生率を比較することを目的とした。

The objective is to compare the risk of postoperative pneumonia after extubation with positive pressure to normal pressure technique. The Department of Anesthesiology at the University of Tsukuba Hospital had been performing a positive pressure technique for extubation after general anesthesia with endotracheal intubation. However, after the outbreak of coronavirus-19 infection, the extubation technique was changed from the positive pressure technique to a normal pressure technique in order to prevent droplet infection during extubation. Although the positive pressure technique is theoretically effective in preventing aspiration pneumonia, no human studies have confirmed its efficacy. Therefore, the purpose of this study was to compare the incidence of postoperative pneumonia between positive and normal pressure techniques.

筑波大学附属病院周術期データベースには2017年4月1日から2022年3月31日までに筑波大学附属病院で全身麻酔、脊椎麻酔、または硬膜外麻酔下の手術をした患者のデータがある。このうち、今回の研究では気管内挿管を伴う全身麻酔を行った患者のデータを用いる。

The perioperative database from the University of Tsukuba Hospital has patients who underwent surgery under general anesthesia, spinal anesthesia, or epidural anesthesia at the University of Tsukuba Hospital between April 1, 2017 and March 31, 2022.
Among them, patients who underwent surgery under general anesthesia with tracheal intubation are included.

除外基準/Key exclusion criteria

日本語
気管内挿管なしの全身麻酔下の手術を受けた患者。
脊椎麻酔または硬膜外麻酔下の手術を受けた患者。
オプトアウトの結果、研究への参加を拒否した患者。

Patients who underwent surgery under general anesthesia without tracheal intubation. Patients who underwent surgery under spinal or epidural anesthesia.
Those who refused to participate in the study as a result of opting out.

本研究は、筑波大学附属病院周術期データベースを用いた前後比較による観察研究である。このデータベースには2017年4月1日から2022年3月31日までに筑波大学附属病院で全身麻酔、脊椎麻酔、または硬膜外麻酔で手術を受けた患者が登録されている。そのうち、本研究では気管挿管を伴う全身麻酔下での手術を受けた患者を対象とする。

筑波大学附属病院麻酔科では、気管内挿管を伴う全身麻酔後の抜管は加圧抜管で行っていた。しかし、コロナウイルス感染症流行後は、抜管時の飛沫感染を予防するために、抜管方法を加圧抜管から平圧抜管に変更した。加圧抜管は理論上誤嚥性肺炎の予防に有効であるが、その有効性を示したヒトを対象とした研究はない。そこで本研究では、加圧抜管と平圧抜管の術後肺炎の発生率を比較することを目的とした。

This study is the before and after observational study using a perioperative database from the University of Tsukuba Hospital. The database has patients who underwent surgery under general anesthesia, spinal anesthesia, or epidural anesthesia at the University of Tsukuba Hospital between April 1, 2017 and March 31, 2022. Among them, patients who underwent surgery under general anesthesia with tracheal intubation are included.
