本研究ではあらかじめ昭和大学横浜市北部病院消化器センターで大腸内視鏡検査を受けた患者のポリープ画像を使用し、内視鏡医が腫瘍性ポリープか非腫瘍性ポリープなのかを読影して診断する。この試験は２回実施され１回目のNBI-CADの診断補助がない場合と２回目の診断補助がある場合の診断精度を比較し、NBI-CADの診断精度上乗せ効果を検証することを目的とする。

In this study, polyp images of patients who underwent colonoscopy at the Digestive Disease Center of Showa University Northern Yokohama Hospital will be used, and endoscopists will read and diagnose whether the polyps are neoplastic or nonneoplastic. This study will be conducted twice to compare the diagnostic accuracy of NBI-CAD without NBI-CAD in the first session and with NBI-CAD in the second session, and to verify the effect of NBI-CAD on the diagnostic accuracy of NBI-CAD.

病変画像には2021年7月から2022年1月までに昭和大学横浜市北部病院消化器センターで大腸内視鏡検査を実施した患者における、病理診断が確定されたポリープの内視鏡画像を使用する。
研究員の用意したスタンドアロンのコンピューター上にあらかじめ設計したサンプルサイズ数の匿名化した病変画像を表示させ、病変画像を見ながら、その病変が腫瘍か非腫瘍かを、その診断の確信度（高確信度or低確信度）とともに入力する。その際にNBI-CADの結果表示はない。読影する医師はサンプルサイズ数に基づき7名の専門医と7名の非専門医とした。1回目の読影終了後から4週間の期間を開けて2回目の読影をNBI-CADの結果表示とともに取り行った。1回目と2回目および各読影医の画像表示順は乱数生成によってランダムに並び替えられる。

本研究で使用されるNBI-CADは、先行研究(昭和大学横浜市北部病院・倫理委員会番号20H048)で開発したAIを使用し、機械学習に用いた画像は2021年6月末日までに撮影された画像である。NBI画像（オリンパス社の狭帯域光観察モード）を入力すると、解析が始まり、病変がNeoplastic（腫瘍）なのか、Non-neoplastic（非腫瘍）なのかを出力することができる。なお画像内に病変が映っていない場合、病変が映っていてもぶれた画像などの場合、病変が映っていても遠方にあり小さく描出されている場合は「Not a good sample」や「Blur image」と表示され支援結果は出力されない。

The lesion images will be endoscopic images of polyps for which the pathological diagnosis has been confirmed in patients who underwent colonoscopy at the Digestive Organs Center of Showa University Northern Yokohama Hospital between July 2021 and January 2022.
The order in which the images were displayed for the first and second reading, as well as for each physician, was randomly sorted by random number generation.

The NBI-CAD used in this study is based on the AI developed in a previous study (Showa University Northern Yokohama Hospital, Ethics Committee No. 20H048), and the images used for machine learning were those taken by the end of June 2021. The NBI image (Olympus narrow-band light mode) can be input and analyzed to determine whether the lesion is neoplastic or non-neoplastic. If there is no lesion in the image, if the lesion is present but the image is blurred, or if the lesion is present but distant and small, "Not a good sample" or "Blur image" will be displayed and no support result will be output.
The researcher will display a predesigned sample size of anonymized lesion images on a stand-alone computer, and while viewing the lesion images, the user will enter whether the lesion is tumor or non-tumor, along with the level of confidence (high confidence or low confidence) of the diagnosis. The NBI-CAD results are not displayed at that time. The physicians reading the images were 7 endoscopists and 7 non-endoscopists based on the number of sample sizes.
