本邦における下大静脈腫瘍栓(Vv3)合併肝細胞癌の切除例を多施設共同で集積し、本邦における下大静脈腫瘍栓(Vv3)合併肝細胞癌の外科治療の短期および長期成績を明らかにするとともに、どのような下大静脈腫瘍栓(Vv3)合併肝細胞癌に手術適応があるのかを明らかにし、下大静脈腫瘍栓(Vv3)合併肝細胞癌に関する提言やガイドラインを作成すること。

The aim of this study is to collect the clinical data on liver resection for hepatocellular carcinoma (HCC) accompanied by tumor thrombus (TT) in the inferior vena cava (IVC) or right atrium (RA) in Japan as a multicenter study, to clarify the short- and long-term outcomes of surgical treatment for these patients, to determine which types of HCC with TT in the IVC or RA are amenable to surgery, and to develop recommendations and guidelines concerning HCC accompanied by TT in the IVC or RA.

本研究の評価項目は，患者背景および術前情報，腫瘍栓のタイプ，手術および術後情報，病理学的情報，ならびに再発および生存データから構成されている。門脈腫瘍栓および胆管腫瘍栓の程度は、日本の肝癌取り扱い規約第6版に従って分類する。肝切除の術式に加えて、下大静脈・右心房腫瘍栓の切除方法、例えば下大静脈壁の同時切除、グラフトの使用、人工心肺の使用に関する情報なども記録する。根治を目的とした手術（R0/1切除）は、手術終了時に肉眼的に残存腫瘍が確認されないものと定義し、それ以外は遺残手術（R2切除）とする。肝不全、胆汁漏、術後出血はInternational Study Group of Liver Surgeryの定義にしたがって記録する。その他の合併症はClavien-Dindo分類に従って記録する。手術関連死（グレードVの合併症）は、退院前の患者の死亡と定義した。

Outcomes of this study consisted of demographic and preoperative information, type of TT, operative and postoperative information, pathologic information, and recurrence and survival data. The extent of portal vein tumor thrombus (PVTT) and bile duct tumor thrombus (BDTT) were categorized according to the Japanese staging system. In addition to providing the name of the surgical procedure for liver resection, information regarding the surgical method for TT in the IVC or RA, such as concomitant resection of the IVC wall, the use of graft, and the use of cardiopulmonary bypass (CPB) was also recorded. Curative-intent surgery (R0/1 resection) was defined as those with no residual tumor observed macroscopically at the end of the surgery, otherwise considered palliative (R2 resection). Liver failure, bile leakage, and postoperative hemorrhage were recorded using the definition of the International Study Group of Liver Surgery. Other complications were recorded according to the Clavien-Dindo classification. Surgery-related death (complications of grade V) was defined as the death of a patient before discharge.

1) 下大静脈腫瘍栓(Vv3)合併肝細胞癌に対して肝切除を行った症例。術前画像検査では下大静脈腫瘍栓を認めなかったが、病理診断で下大静脈腫瘍栓(Vv3)合併肝細胞癌であった症例を含む。
2) CT, US, MRIのうちいずれかのmodalityで診断をしている症例。
3) 開腹あるいは腹腔鏡下に切除を行った症例。
4) 臨床研究参加について、院内の包括的同意書によって患者本人および家族から、十分な理解と文書による同意が得られている。

1) Patients who underwent liver resection for HCC with TT in the IVC or RA. This includes cases of HCC with TT in the IVC or RA on pathological diagnosis, even though no TT in the IVC or RA was found in imaging studies before surgery.
2) Cases in which the diagnosis was made by one of the modalities of CT, US, or MRI.
3) Patients who underwent open or laparoscopic resection.
4) Patients and their family members have given full understanding and written consent for participation in clinical research by means of the hospital's comprehensive consent form.

除外基準/Key exclusion criteria

日本語
1) 内科的治療（肝動脈塞栓術、化学療法等）のみを行い、肝切除を行っていない症例。
2) 術前診断は肝細胞癌であったが、切除標本では他の腫瘍性病変だった症例。
3) その他担当医師が不適当と判断した患者。

1) Cases in which only medical treatment (transcatheter arterial chemoembolization, chemotherapy, etc.) was performed and liver resection was not performed.
2) Patients whose preoperative diagnosis was HCC, but the resected specimen showed other neoplastic lesions.
3) Other patients deemed inappropriate by the physician in charge.

本後ろ向き多施設調査は、日本肝胆膵外科学会のプロジェクト研究として企画されたものである。本調査は、東京大学倫理委員会（審査番号：11588）および各参加施設の承認を得て実施する。日本肝胆膵外科学会に登録されている高度先進外科研修施設 211 施設のうち、下大静脈または右心房に腫瘍栓を有する肝細胞癌の手術経験を有する 23 施設が本研究に参加する。前述の評価項目について参加施設にアンケート用紙を送付し、返信結果を東京大学肝胆膵外科で集計・分析する。

This retrospective, multi-center survey was planned as a project study of the Japanese Society of Hepato-Biliary-Pancreatic Surgery. This study was conducted with the approval of the ethics committee of the University of Tokyo (Review number: 11588) and of each participating institution. Among the 211 training institutions for highly advanced surgery registered by the Japanese Society of Hepato-Biliary-Pancreatic Surgery, 23 institutions with experience in surgery for HCC with TT in the IVC or RA participated in this study. Questionnaire sheets were sent to the participating institutions and the results of the replies were collected and analyzed at the Hepato-Biliary-Pancreatic Surgery Division, the University of Tokyo.
