| 目的1/Narrative objectives1 | 本研究は、医療ビッグデータであるDPCデータベースを利用し、神経発達症である注意欠如・多動症（以下、ADHD）ならびに自閉スペクトラム症（以下、ASD）の成人期患者における併存疾患の実態を評価し、得られた結果を日本の先行研究と比較することで、DPCデータが神経発達症の実臨床の実態をどの程度反映しているのかを検討する。 | This study will evaluate the actual status of comorbidities in adult patients with neurodevelopmental disorders such as attention-deficit/hyperactivity disorder (ADHD) and autistic spectrum disorder (ASD) using the DPC database, a big data set for medical care, and compare the results obtained with previous Japanese studies to determine to what extent the DPC data reflect the actual clinical status of neurodevelopmental disorders. |

| 目的2 -その他詳細/Basic objectives -Others | 今回の研究では、日本のビックデータであるDPCデータを活用して発達障害患者における併存疾患の実態を把握し、先行研究と比較する。その乖離の程度を把握することで、DPCデータが神経発達症患者の調査を実施するに耐えうるデータベースであるか否かを検討する。
成人期発達障害患者では、発達障害そのものの症状で来院するというよりは、むしろ併存症の症状を訴えて来院するケースのほうが多いことが容易に推測される。そのような患者は、発達障害としても重症である可能性が高いので、併存症の背景にある発達障害を見逃さずに診断することが重要となる。そのため、成人期発達障害患者にどのような併存疾患が合併しやすいかを明確にすることで、ASD/ADHDの適正診断の一助になることが期待できる。 |
In this study, we will utilize DPC data, which is Japanese big data, to understand the actual status of comorbidities in patients with developmental disabilities, and compare it with previous studies. By understanding the degree of discrepancy, we will examine whether the DPC data is a database that can withstand investigation of patients with neurodevelopmental disorders.
It is easy to assume that patients with developmental disorders in adulthood tend to present with symptoms of comorbidities rather than with symptoms of developmental disorders themselves. Since such patients are likely to have severe developmental disorders, it is important to diagnose them without overlooking the developmental disorders underlying their comorbidities. Therefore, clarifying which comorbidities tend to be associated with developmental disorders in adult patients with developmental disorders is expected to help in the proper diagnosis of ASD/ADHD. |

| 主要アウトカム評価項目/Primary outcomes | 研究の目的は、
①DPCデータベースを用いたADHD患者の併存疾患の実態を把握する ②DPCデータベースを用いたASD患者の併存疾患の実態を把握する ③DPCデータベースによる調査結果と日本における先行研究との比較を実施し、DPCデータベースを活用した神経発達症の実態調査の妥当性を検討する 以上3点である。 <データの取得方法> 調査データ： DPCデータ調査研究班（厚生労働科学研究伏見班）から第三者提供により過去 5 年分の DPCデータの提供を受ける。対象者は、全国のすべてのDPC調査対象病院（約1600施設）での症例とする。 対象施設： DPC対象病院約 1600 施設のうち、本研究班へのデータ提供の同意が得られ、一般社団法人診断群分類研究支援機構にDPC調査データの提供を行った施設 対象者： DPC 調査データに含まれ、2016年 4月～2020年3月における成人期(18歳以上)のADHD/ASD症例 取得するデータ： DPC調査データ；様式 1、様式 3、様式 4、入院・外来 EF ファイル・Dファイル 抽出方法： [ADHD/ASD]は以下コードから抽出する。（病名：ICD10コード） 『ADHD』⇒F90：多動性障害 (注意欠陥多動障害：F900，多動性素行障害：F901，小児行動異常/多動性障：F909) 『ASD』⇒F84.9：広汎性発達障害，詳細不明(広汎性発達障害：F849，自閉症スペクトラム障害：F849) |
The objectives of the study are
(1) To understand the actual status of comorbidities in ADHD patients using the DPC database (2) To understand the actual status of comorbidities in ASD patients using the DPC database. (3) To compare the results of the survey using the DPC database with previous studies in Japan, and to examine the validity of the survey of the actual conditions of neurodevelopmental disorders using the DPC database. These are the above three points. <Data Acquisition Method> Survey data: DPC data for the past five years will be provided by a third party from the DPC Data Research Group (Fushimi Group of the MHLW Science Research). Subjects will be cases at all DPC survey hospitals in Japan (approximately 1,600 facilities). Target facilities: Of the approximately 1,600 DPC hospitals, those that have agreed to provide data to the study group and have provided DPC survey data to the Diagnostic Group Classification Research Support Organization. Subjects: ADHD/ASD cases in adulthood (age 18 or older) from April 2016 to March 2020 included in the DPC survey data Data to be obtained: DPC survey data; Form 1, Form 3, Form 4, inpatient and outpatient EF and D files Extraction method: [ADHD/ASD [ADHD/ASD] is extracted from the following codes. (Disease name: ICD10 code) ADHD F90: Hyperactivity disorder (Attention deficit hyperactivity disorder: F900, Hyperactivity disorder: F901, Childhood behavioral disorder/hyperactivity disorder: F909) ASD F84.9: Pervasive developmental disorder, details unknown (Pervasive developmental disorder: F849, Autism spectrum disorder:F849) |
