地域在住高齢者に対し，訪問リハビリテーションによる生活行為向上マネジメントが心身機能，ADL，IADL，およびQOLに影響を及ぼすか効果を検証すること．

地域在住の高齢者に対し，生活行為マネジメントにてリハビリテーションの介入を決定し，週1～2回の頻度で12か月間，訪問リハビリテーションを実施する．

The Management Tool for Daily Life Performance will be used to determine life-focused interventions for elderly people living in the community dwellings and interventions by home-visit rehabilitation will be conducted once or twice a week for 12 months.

選択基準/Key inclusion criteria

新規に訪問リハビリテーションを開始する者
同意取得時において年齢が65歳以上の者
本研究への参加にあたり十分な説明を受けた後、患者本人又は代諾者の自由意思に基づき文書による同意が得られた者

Individuals who newly start home-visit rehabilitation
Individuals who are 65 years of age or older at the time of obtaining consent
Individuals who have received sufficient explanation of their participation in this study and have obtained written consent based on the free will of the participant or a surrogate.

除外基準/Key exclusion criteria
