目的1/Narrative objectives1

日本語
シャント増設術において、術野が上腕・腋窩に及ぶ場合や術野が広範囲になる可能性がある場合、患者の疼痛が強いことが予想されるが全身状態が悪く全身麻酔を避けたい場合などに、心臓血管外科より依頼があり、伝達麻酔を行っている。伝達麻酔を行うと交感神経も遮断され、それに伴い術野の動静脈が拡張してくると考えられる。シャント増設術はなるべく径が太い皮静脈に動脈を吻合する手技であり、伝達麻酔を施行することで術野の皮静脈が拡張するのであれば、伝達麻酔に鎮痛効果の他にシャント増設術を施行しやすくなる副次的効果が認められる可能性がある。そこで、シャント増設術の伝達麻酔管理症例において、伝達麻酔前後の皮静脈血管径を測定し比較検討を行いたい。

In shunting procedures, a cardiovascular surgeon requests a nerve block when the surgical field extends into the upper arm or axilla, when the surgical field may be extensive, or when the patient is in poor general condition and general anesthesia should be avoided, although the patient is expected to be in a lot of pain. It is thought that the sympathetic nerves are also blocked when transfer anesthesia is performed, and the arteriovenous system in the operative field is dilated accordingly. If the nerve block causes the arteries in the operative field to dilate, it may have a secondary effect of facilitating the shunt procedure in addition to its analgesic effect. Therefore, we would like to measure and compare the diameters of the vascular vessels in patients who underwent shunt augmentation before and after the nerve block anesthesia.

シャント造設術をうける患者で、伝達麻酔にて行うもの

除外基準/Key exclusion criteria

日本語
伝達麻酔の効果が不良であった患者（術野での疼痛が強く局所麻酔以外に鎮痛薬の静脈投与や鎮静薬の投与を必要とした患者、全身麻酔に移行した患者）。気胸や鎖骨下動脈穿刺など伝達麻酔施行による合併症が生じた患者。循環動態不良で血管収縮薬を使用している患者。

Patients in whom the effects of nerve block were poor (patients with severe pain in the operative field requiring intravenous analgesics or sedatives in addition to regional anesthesia, or patients who were converted to general anesthesia). Patients with complications from the administration of nerve block, such as pneumothorax or subclavian artery puncture. Patients on vasoconstrictors for poor circulation.
