目的/Objectives

日本語
本研究では、心房細動と診断された職域健診受診者に対して、新たな受診勧奨の方法を導入することにより未受診率を低下させることを目的とする。その達成により、未治療者に対して適切な治療に誘導することで、県内の心房細動患者の治療の向上を図る体制構築に結び付くものと思われる。健診機関と実地医家、病院の有機的な関係を構築し、新潟県における循環器病対策の中心の一つなるプロジェクトとして発展させていきたい。

The purpose of this study is to reduce the non-examination rate by introducing a new method of recommending examinations for those who have undergone occupational medical examinations who have been diagnosed with atrial fibrillation. Achieving this will lead to the establishment of a system to improve the treatment of patients with atrial fibrillation in the prefecture by inducing appropriate treatment for untreated patients. We would like to build an organic relationship between medical examination institutions, on-the-job doctors, and hospitals, and develop it as one of the central projects for cardiovascular disease control in Niigata Prefecture.

2018年12月に「健康寿命の延伸等を図るための脳卒中、心臓病その他の循環器病に係る対策に関する基本法」（以下「循環器病対策基本法」）が成立した。同法に基づき循環器病対策推進基本計画が2020年10月に閣議決定され、各都道府県がその地域事情に合わせた脳卒中・循現器病に対する都道府県計画を策定していく事となった。2021年に新潟県循環器病対策推進協議会が設立され、そのなかで新潟県内における心房細動治療に対する健診機関を含めた病診連携を再構築し、県民の健康増進を図る事が確認された。

健診で心房細動の心電図を有し、翌年の特定健診質問票の問5「心臓病」の項目で「治療中」にチェックをつけた受診者数を、心房細動を指摘され翌年も受診した人数で除して治療率を計算する。

未治療者の心房細動受診者（下記の既往・現病歴にて、チェックが無い人、及び既往歴にチェックのある人で、心電図上心房細動と診断された人）を対象とする。

健診結果郵送時に受診を促す手紙を同封し、発送後3ヶ月後に紹介状返事が新潟県労働衛生医学協会に来ていない場合、企業の衛生管理者に連絡し再度受診を促す。
