本介入プロトコルは、多職種によるInterdisciplinary modelに基づいた包括的支援プログラムである。直接毎日のケアに関与する専門職種の看護師、介護士、理学療法士、作業療法士、言語聴覚士は、専門知識やスキルが専門分野間で重複していることがあり、専門職間で密なコミュニケーションを図り、場合によって学問の境界を越え、患者の訴えるニーズと目標に沿った実践的な解決策を提案する。

This intervention protocol is a comprehensive support program based on an Interdisciplinary model. Professional nurses, caregivers, physical therapists, occupational therapists, and Speech-Language Pathologists, who are directly involved in the daily care of patients, may have overlapping expertise and skills between disciplines, and will work closely with each other, in some cases across disciplinary boundaries, to develop practical solutions that are in line with the patients' expressed needs and goals.

・MDSのPD診断基準を満たしている者
・書面による同意が得られる者

-Those who meet PD diagnostic criteria for MDS.

-Those who can give written consent.

除外基準/Key exclusion criteria

日本語
・視覚・聴覚に障害があり、研究への参加に制限をきたす場合
・認知機能（MMSEが8/30以下）が低下している場合
・レヴィ－小体型認知症である場合

Visual or hearing impairment that would limit participation in the study
Cognitive function (MMSE less than 8/30) is impaired.
