目的1/Narrative objectives1

日本語
大腿骨近位部骨折（HF）は後期高齢者に頻発する骨粗鬆症性骨折で，その90％以上は手術療法が適用され，術後はADLの改善を目指し，関節可動域訓練や筋力トレーニング，歩行練習などの標準的リハビリテーション（リハ）アプローチが実践される．しかし，これだけでは地域で快適に暮らせるだけの身体機能や身体活動量の向上には繋がっていないのが現状である．そして，最近では，身体活動量の向上を促すには共有意思決定（Shared decision making；SDM）に基づいた目標設定を組み込んだ行動医学的介入が有用とされている．そこで，本研究ではHF術後患者の身体機能やQOLの改善に寄与するSDMに基づいた行動医学的介入を併用した新たなリハ戦略を開発する．

Hip fractures are osteoporotic fractures that occur frequently in the elderly, and more than 90% of them are selected for surgery. After surgery, standard rehabilitation such as range of motion training, strength training, and walking exercises are practiced with the aim of improving ADL. However, the current situation is that the improvement of physical function and physical activity to live comfortably in the town is insufficient. Recently, it has been said that cognitive-behavioral intervention based on shared decision-making is effective in promoting improvement in physical activity. Therefore, in this study, we examine the effect of combined cognitive-behavioral intervention based on shared decision-making that contributes to the improvement of physical function and QOL of patients after hip fracture.

受傷前に屋内歩行が独歩またはT字杖にて自立している者
日本語を理解できる者
研究の参加に同意した者

Injured hip fracture
Those who lived independently by walking alone or walking with a T-shaped cane before the injury
Those who can understand Japanese
Those who have agreed to participate in the study

除外基準/Key exclusion criteria

日本語
高所からの転落などの高エネルギーでの受傷した者
術後2週後のMMSE23点以下の認知機能の低下を認める者
術後4週間以内に退院した方，高度の視力障害を持つ者
リハを妨げる疾患・障害を合併している者
