本研究の目的は，目標志向型のリハビリテーションアプローチを提供されることにより，主観的な日常生活動作や身体活動量の指標と同様に，簡便に使用できるリストバンド型の活動量計で計測された客観的な身体活動量にも改善が認められるのかを検証する．

The aim of this study is to verify whether there is an improvement in the objective physical activity measured by a wristband-type physical activity tracker that is easy to use, as well as an index of subjective activities of daily living and physical activity by providing a goal-oriented rehabilitation approach.

介入前の1週間に自記式調査とリストバンド型活動量計を用いた活動量の評価を行う．
対象者の目標とする日常生活活動を明確にするための面接を行う．
週1回以上の目標志向的なリハビリテーション支援を3カ月間実施する．

In the week before the intervention, the amount of physical activity are assessed by a self-rating survey and a wristband-type physical activity tracker.
Interview to clarify the goals of participant's activities of daily living.
Goal-oriented rehabilitation support will be provided at least once a week for 3 months.

選択基準/Key inclusion criteria

発症，または受傷後の急性期を脱し，病状が安定している状態で，リハビリテーションサービスを週1回以上利用している者．

Those who have survived the acute phase after onset or injury and have stable medical conditions, and use rehabilitation services at least once a week.
除外基準/Key exclusion criteria
日本語
著明な認知機能の低下によって，自己評価が困難であったり，リストバンド型活動量計の管理が困難（紛失，異食など）な者．
不快感やアレルギー反応などによってリストバンド型活動量計の着用継続が困難な者．

It is difficult to self-rating of survey and to manage wristband-type physical activity tracker due to salient decline in cognitive function.
It is difficult to continue wearing the wristband-type physical activity tracker due to discomfort or allergic reaction and so on.
