目的/Objectives

日本語
重篤な呼吸器合併症に罹患したり、気道確保が困難になると気管切開を行うことがある。昨今、新型コロナウイルスによる重症化患者においても、気管切開を行って治療をしていくことが多くなった。気管切開は、局所麻酔で行い、患者自身の呼吸を維持しながらの施行が望ましいとされてきた。しかし、新型コロナウイルスによるエアロゾル感染が報告されるようになり、全身麻酔下での気管切開が感染対策としては安全であるという風潮になっている。
全身麻酔下で気管切開を行う際は、チューブのカフを破かないようにすることが感染対策でも安全面でも重要である。現在、チューブのカフを守るテクニックは、触診法とPressure control ventilation法（以降、PCV法）がある。これらの方法のどちらがより安全で、だれでも確実にできる方法なのかは検討されていない。
従来の方法である、触診法は施行する医師の感覚を基に行われているため、経験が必要とされる可能性があった。そのため、手技の正確さが初心者の場合、ばらつく可能性がある。

A tracheotomy may be performed if severe respiratory complications occur or if airway management becomes difficult. In recent years, even in patients who have become severely ill due to the new coronavirus, tracheostomy is often performed for treatment. It has been considered desirable to perform tracheostomy under local anesthesia and maintain the patient's own breathing. However, aerosol infections caused by the new coronavirus have been reported, and the trend is that tracheostomy under general anesthesia is safe as an infection control measure.
When performing a tracheostomy under general anesthesia, it is important not to break the cuff of the tube in terms of infection control and safety. Currently, there are palpation method and Pressure control ventilation method (hereinafter referred to as PCV method) as techniques for protecting the cuff of the tube. It has not been considered which of these methods is safer and more reliable for everyone.
