本研究では、軽症頭頸部外傷で救急外来を受診した成人に対する頭部頚椎CT撮像の必要性を判断する臨床予測ルールの妥当性検証を日本国内で施行する。それに加えて新たな適応年齢を模索し高齢者へのCT撮像を避けられないか、抗血栓薬内服中の患者にもルールが適応できるのかといった新規の見地を開拓することで、重篤な外傷を見逃すことなく不要な頭部・頚椎CT検査を安全に削減することを目的とする。

The objective of this study is validation a clinical prediction rule and new threshold for determining the necessity of CT imaging in adults who present to the emergency department for minor head and neck trauma in Japan.

既存の臨床予測ルールの妥当性研究だけでなく、新たな適応年齢を模索することで高齢者へのCT撮像を避けられないか、抗血栓薬内服中の患者にもルールが適応できるのかといった新規の見地を開拓することで、重篤な外傷を見逃すことなく不要な頭部・頚椎CT検査を安全に削減することを目的とする。

The aim of this study is to safely reduce unnecessary head and cervical spine CT examinations without missing serious trauma by not only studying the validity of existing clinical prediction rules, but also developing new threshold, such as whether CT imaging in the elderly can be avoided by searching for new indications for age, and whether the rules can be applied to patients on antithrombotic medication.

頭頸部の外傷患者で本研究に参加する医療機関を受診した者のうち以下の症例を除外した集団とする

The following cases will be excluded from the population of patients with head and neck trauma who visited the medical institutions participating in this study

除外基準/Key exclusion criteria

(ア) GCS13点未満の重症頭部外傷患者
(イ) 頭部以外に Abbreviated Injury Scale > 2の外傷がある患者
(ウ) 頭蓋内損傷を伴う頭部外傷,頭蓋骨および頭蓋内手術の既往歴
(エ) 鋭的外傷患者
(オ) 妊婦
(カ) 研究参加を拒否した患者

(a) Patients with severe head injury with GCS <13
(b) Patients with trauma other than head injury with Abbreviated Injury Scale > 2
(c) past medical history of head trauma with intracranial injury, history of skull and intracranial surgery
(d) penetrating trauma
(e) Pregnancy
(f) Patients who refused to participate in the study
