主要アウトカム評価項目/Primary outcomes
日本語
患者は冠動脈造影検査前にカフェイン群と非カフェイン群に任意に分ける。カフェイン群においては、造影検査前にコーヒー(222㎎）を摂取し、冠微小動脈を収縮させる。両群において冠動脈造影上中等度以上の狭窄病変に対し圧検査を実施する。アデノシン経静脈投与によるFFR測定後、ニコランジルを冠動脈内投与して最大充血とする。得られた圧検査値と冠動脈末梢圧(Pd)曲線の特徴を解析し、冠微小循環系の性状を評価する。

Patients group are divided into caffeine and non-caffeine group. Patients in the caffeine group take coffee (caffeine 222mg) before coronary angiography to make coronary microvessels spastic. In middle to severe stenotic lesions on the coronary angiogram, we conduct coronary pressure test. After FFR test using intravenous adenosine infusion, we add intracoronary nicorandil injection to induce maximal hyperemia. We evaluate the coronary microcirculatory condition using the coronary pressure test data and the character of distal coronary pressure curves.
