目的/Objectives

日本語
皮膚筋炎（Dermatomyositis 以下 DM）に間質性肺病変が高率に合併するということが知られており、中でも抗MDA-5抗体陽性例では急速に間質性肺病変が進行し呼吸不全に陥り、治療抵抗性で予後不良な転帰をとること多い。現在は診断後すぐにステロイド＋免疫抑制剤にて積極的に治療介入を行うが、定められたプロトコールが存在するわけでもなく、一律に行っているのが実情である。
また、予後予測因子として診断時のフェリチン値、KL-6値、CTでするガラス陰影の広がり、CRP値などが報告されているが1-12)、治療反応性と予後との関連を見た報告はない。治療抵抗死亡群では、治療開始後3ケ月以内での死亡例が多く、この期間の画像、酸素化、血清LDH、CRP値の変動が生存率に寄与しないか検討を行い、治療変更の必要性について検討する。さらに、症例においては過剰な治療を行っている可能性もあり、経過を検討することで途中減量できる可能性がないか検討する。

anti-MDA5 antibody positive dermatomyositis shows very poor outcome because it always complicates interstitial lung disease. Many prognostic factors have already reported, for example initial serum ferritin or KL-6 level. But believe that annual change of inflammation marker or KL-6 is correlate with outcome of this disease.

選択基準/Key inclusion criteria
日本語
久留米大学病院呼吸器・神経・膠原病内科で、平成12年1月1日～令和4年1月31日までの間に皮膚筋炎と診断された患者の診療情報を使用する。その中で抗MDA-5抗体陽性例は35例であり、発症時の臨床症状や検査データは参加拒否の方以外全例登録し、毎週の血液データと酸素化を確認し、6ケ月後の生存確認が可能な抗MDA-5抗体陽性例20例である。

This study included 85 consecutive patients diagnosed with PM/DM between 2002 and 2020 at the Kurume University Hospital. Clinical data, cumulative disease manifestations, laboratory investigations, associated diseases, therapy, clinical course, disease complications, and outcomes were retrospectively recorded from medical records.

本試験で用いる内服薬（ステロイド剤、免疫抑制剤）によって症状の増悪するリスクのある重い合併症[糖尿病：（血糖値上昇、免疫力低下、易感染性のリスク等）、心臓病：（免疫力低下、易感染性のリスク等）、感染症：（免疫力低下、易感染性のリスク等）、骨髄抑制、肝機能障害、腎機能障害、肺障害（間質性肺炎等）、がん性疾患）]を持っている患者
研究責任者及び研究分担者が研究対象者として不適当と判断した方
