目的/Objectives
目的1/Narrative objectives1	食物アレルギーは、原因食物を摂取してから 1時間以内に皮膚症状（蕁麻疹等）や呼吸器症状（くしゃみ、咳、喘鳴）を認める 即時型アレルギーがよく知られている。一方で、原因食物を摂取してから数時間以降に主に消化器症状（嘔吐や下痢等）を認める食物蛋白誘発胃腸炎が存在し、近年患者数が増加していることが知られている。(Nowak-Wegrzyn A. et al. JACI,2017) 現時点での標準的治療は、原因食物の除去を指導し、耐性獲得の確認は経口食物負荷試験（OFC; oral food challenge）を行うことである。即時型アレルギーと同様に成長過程で耐性獲得することは知られているが、遷延する症例も存在し、即時型アレルギーと比較して、食物蛋白誘発胃腸炎の臨床的特徴は不明な点が多い。食物のどういった成分に反応し、どの様な機序で耐性獲得するのかは明らかになっていない。 鶏卵の即時型アレルギーに対するOFCでは、標準化抗原として市販されている乾燥加熱全卵粉末の使用が、ガイドラインでも推奨されている。食物蛋白誘発胃腸炎に対するOFCの標準的な指針はなく、各施設によって異なる。 当院では卵黄による食物蛋白誘発胃腸炎患者に対し、20分以上加熱した固形卵黄を用いてOFCを用いて行なっているが、本研究では、乾燥加熱全卵粉末を用いてOFCを行い、その有用性を評価したい。	The most common type of food allergy is immediate allergy, in which skin and respiratory symptoms are observed within an hour of ingestion of the causative food. On the other hand, there is a type of food protein-induced enterocolitis (FPIES) in which gastrointestinal symptoms are mainly observed within a few hours after ingestion of the causative food, and it is known that the number of patients has been increasing in recent years. At present, the standard treatment is to teach the elimination of the causative food, and to confirm the acquisition of tolerance by conducting an oral food challenge (OFC) test. As with immediate food allergy, it is known that tolerance is acquired during the growth process, but there are also cases of prolonged tolerance. The clinical features of FPIES are much less clear than those of immediate food allergy. It is still unclear what components of the food react with the protein and what mechanisms are used to develop tolerance. In the OFC for immediate type food allergy to hen's eggs, the use of dried cooked whole egg powder, which is commercially available as a standardized antigen, is recommended in the guidelines. There is no standard guideline for OFC for FPIES, and it varies from institution to institution. In this study, we will evaluate the efficacy of OFC using dried and heated whole egg powder.

選択基準/Key inclusion criteria	当院で卵黄によるsolid FPIESの診断を受けた患者で、加熱卵白に対する即時型食物アレルギーがない患者	Patients who have been diagnosed with solid FPIES due to egg yolk at our hospital and do not have an immediate food allergy to heated hen's egg white
除外基準/Key exclusion criteria	卵白摂取が確認できていない患者 卵白による即時型食物アレルギーの合併がある患者	Patients with unconfirmed egg white intake Patients with complications of immediate food allergy to egg white
