肝門部領域胆管癌、高リスク症例に対する肝切除を伴う根治手術を行った症例において、術後合併症および予後情報を確認し、本邦における高リスク肝門部胆管癌症例に対する手術成績を明らかにする。

選択基準/Key inclusion criteria

①肝切除を伴う根治手術を施行した症例
②Bench Mark Studyにおいて高リスク症例と判断された症例
以下に示す高リスク因子が一つでも当てはまる症例
血管合併切除（肝動脈・門脈・下大静脈）施行
膵頭十二指腸同時切除施行
術前抗腫瘍療法施行
American Society of Anesthesiologists (ASA) 分類 >3
Body mass index 35 kg/m2以上
心疾患の既往（心房細動・心臓手術：冠動脈形成術を含む）
腎機能障害；GFR<60 mL/min/1.73 m2 or serum creatinine >1.8 mg/dL
呼吸器疾患：１秒率８０％未満
血管病変：抗凝固剤内服中
糖尿病：２剤以上の糖尿病薬内服またはインスリン使用

③本研究の参加について拒否しない者

（1）研究の種類・デザイン
既存の情報を用いる多施設共同観察研究
（2）調査項目
研究対象者について、2026年1２月31日までの診療情報を収集し、以下の項目の調査をおこなう。

①　背景情報：年齢、性別、病歴、診断名、治療歴、Performance Status
②　血液学的検査結果：ヘモグロビン、白血球数、白血球分画・血小板数
③　血液生化学的検査結果：総蛋白、アルブミン、Na、K、血清腫瘍マーカー（CEA/CA19-9）
④　CT・MRI・超音波・胆管造影画像検査結果
⑤　病理組織学的検査結果（組織型・進行度・リンパ節転移）
⑥　施行術式・手術時間・出血量
⑦　術後経過（合併症有無）
⑧　術後治療種類・期間。
⑨　再発までの期間・再発部位・生命予後・最終通院日・死亡原因

(1) Type and design of the study
Multicentre observational study using existing information
(2) Study items
Medical information on the study subjects will be collected until December 31, 2026, and the following items will be investigated.

(1) Background information: age, sex, medical history, diagnosis, treatment history, performance status
2) Hematological examination results: hemoglobin, white blood cell count, white blood cell fraction and platelet count
3) Blood chemistry test results: total protein, albumin, Na, K, serum tumor marker (CEA/CA19-9)
4) CT, MRI, ultrasound and cholangiogram results
5) Results of histopathological examination (histological type, degree of progression, lymph node metastasis)
(6) Operative procedure, operative time, amount of blood loss
7) Postoperative course (presence of complications)
(viii) Type and duration of postoperative treatment.
(viii) Type and duration of postoperative treatment (ix) Time to recurrence, site of recurrence, prognosis of life, date of last hospital visit, cause of death
