During the 14-day hospitalization period, each patient received 18 treatment sessions consisting of 20-min low-frequency repetitive transcranial magnetic stimulation and 120-min intensive occupational therapy.

選択基準/Key inclusion criteria
日本語
（1）介入時の年齢が18歳以上かつ80歳以下．
（2）脳卒中発症から1年以上経過している．
（3）軽症～中等症の上肢麻痺（Action Research Arm Testが16点以上）
（4）従来の作業療法を施行されてきたが機能回復はﾌﾟﾗﾄｰと判断されている．
（5）脳卒中の既往が1回のみで病巣が皮質下に限局している．

1) age at the time of intervention of 18-80 years.
2) time after stroke onset of greater than 12 months.
3) mild-to-moderate upper limb hemiparesis (Action Research Arm Test score >= 16).
4) a plateau in maximum motor recovery after conventional rehabilitation.
5) a history of single subcortical stroke only.
除外基準/Key exclusion criteria
日本語
（1）明らかな認知機能障害がある．
（2）積極的な医学的処置を必要とする活動性の全身性疾患もしくは精神疾患に罹患している．
（3）過去1年間において痙攣発作の既往がある．
（4）Wassermann（1998年）が提唱したｶﾞｲﾄﾞﾗｲﾝに挙げられている禁忌項目がある．

1) cognitive deficits (Mini-Mental State Examination score <= 23).
2) active systemic or mental illness requiring medical management.
3) seizure.
4) pathological conditions known to be contraindications for rTMS in the guidelines suggested by Wassermann (1998).

主な結果/Results
日本語
上肢機能検査では，治療後に統計学的に有意な成績改善が認められた．fMRIでは，多くの症例において，治療前には麻痺手運動時に両側運動野の広範な活動が認められたが，治療後には活動が病変側に収束し，健常者の活動ﾊﾟ ﾊﾟ ﾀｰﾝに近づくこれに対応し，一次運動野・一次感覚野・運動前野において治療後に有意なLIの上昇が認められた．さらに，LI変化量と上肢機能検査改善度との間に有意な相関が認められた．

After the treatment, the motor function test scores of FMA and WMFT improved significantly. The LI during movements of the affected hand increased significantly, suggesting changes in the activation balance within the primary motor, primary lesioned hemisphere. Moreover, the LI changes in the aforementioned areas significantly correlated with gains in the WMFT.
