目的1/Narrative objectives1	近年、腹腔鏡下手術は増加の一途を辿っている。特に下部消化管や泌尿器科ダビンチ手術等は、骨盤内臓器のため、術中視野確保に著しい頭低位を取らなければならない。これらの体位は心血管系に与える影響も少なくなく、左室駆出率（LVEF）が2割低下するとの報告がある1）。特に心疾患合併患者はその変化が著しくなることが予想される。そこで、健常人ボランディアを対象に、左心系、右心系の心収縮力や拡張能が頭低位によって、どれだけ変化するかを調査研究することとした。	In recent years, laparoscopic surgery has been increasing steadily. In particular, the lower gastrointestinal tract and urological Da Vinci surgery are organs in the pelvis, so it is necessary to take a remarkable low head position to secure the intraoperative visual field. It has been reported that these positions have a considerable effect on the cardiovascular system, and the left ventricular ejection fraction (LVEF) is reduced by 20% 1). Especially in patients with heart disease, the change is expected to be remarkable. Therefore, we decided to investigate and study how much the myocardial contractile force and diastolic ability of the left and right heart systems change depending on the low head position in healthy human volunteers.

評価/Assessment
主要アウトカム評価項目/Primary outcomes	ボランティア30名に対し、先ず水平位で左右収縮能、拡張能を経胸壁心エコーにて測定する。 収縮能は左右室駆出率（Ejection Fraction: EF）、短縮率（Fractional Shortening: %FS）, そして測定が容易でEFと良く相関するといわれている僧帽弁、三尖弁輪収縮期移動距離（MAPSE, TAPSE）2)を測定する。拡張能はE/A, E/e’を左心、右心系3）どちらも測定を行う。その後、頭低位30度を7.5分間4）保ち、上記収縮能、拡張能の変化を記録する。また、血行動態として水平位、頭低位での血圧、心拍数を記録する。 本研究の目的は一般的測定値であるEF, %FS, E/Aと比較的新しいMAPSE, TAPSE, E/e’がどのように相関するか？そしてこれらの値が頭低位によってどう変化するかを検証することにある。	For 30 volunteers, the lateral contraction ability and diastolic ability are first measured by transthoracic echocardiography in the horizontal position. And it is to examine how these values change depending on the low head.
