目的/Objectives
目的1/Narrative objectives1
日本語
1) 運動トレーニングを中心に構成された心臓リハビリテーション介入がHFpEF患者の運動耐容能、左室構造機能、最大下運動中の血行動態に与える影響を明らかにすること
2) 運動トレーニングを中心に構成された心臓リハビリテーション介入がHFpEF患者の運動耐容能 (peakVO2および6MWD)の変化に及ぼす影響を評価し、2型糖尿病の合併がこれらの変化に影響を及ぼすかどうかを評価する

1) To clarify the effects of exercise training based cardiac rehabilitation on exercise capacity, left ventricular structure and function, and hemodynamics in patients with HFpEF
2) To evaluate the effect of cardiac rehabilitation intervention centered on exercise training on changes in exercise tolerance (peak VO2 and 6MWD) in patients with HFpEF, and to assess whether the presence of type 2 diabetes mellitus affects these changes

選択基準/Key inclusion criteria

New York Heart Association (NYHA)の機能分類 II または III
左室駆出率が保持された心不全患者
・臨床的な心不全症状
・左室駆出率>50%
・左室拡張機能障害 (mean E/e′ >14, septal e′ <7 cm/s または lateral e′ <10 cm/s, peak tricuspid regurgitation velocity >2.8 m/s, LAVI >34 mL/m2のうち3つ以上を満たす場合)
・HFA-PEFF score >5点
2型糖尿病
・空腹時血糖 ≧126 mg/dL
・75g経口ブドウ糖負荷試験の2時間値 ≧ 200 mg/dL
・随時血糖 ≧200 mg/dL
・Hemoglobin A1c ≧6.5%.

Heart failure with preserved ejection fraction
Clinical symptoms of heart failure
Left ventricular ejection fraction>50%
Left ventricular diastolic dysfunction as defined by the American Society of Echocardiography/European Association of Cardiovascular Imaging
HFA-PEFF score >5 points
Type 2 diabetes mellitus
Fasting plasma glucose level was >126 mg/dL
2 h value for the 75 g oral glucose tolerance test was >200 mg/dL
Causal plasma glucose level was > 200 mg/dL
Hemoglobin A1c level > 6.5%
除外基準/Key exclusion criteria
日本語
1.過去1週間以内における心不全の自覚症状（呼吸困難、易疲労性など）の増悪
2.不安定狭心症または閾値の低い（平地ゆっくり歩行［2METs］で誘発される）心筋虚血
3.手術適応のある重症弁膜症、特に大動脈弁狭窄症
4.重症の左室流出路狭窄（閉塞性肥大型心筋症
5.未治療の運動誘発性重症不整脈（心室細動、持続性心室頻拍）
6.活動性の心筋炎
7.急性全身性疾患または発熱
8.運動療法が禁忌となるその他の疾患（中等症以上の大動脈瘤、重症高血圧、血栓性静脈炎、2週間以内の塞栓症、重篤な他臓器障害など）
9.先天性の心血管疾患患者
10.推算糸球体濾過量<30の腎不全
11.精神科系疾患の診断を受けている者
12.悪性腫瘍
13.NYHA分類〈New York Heart Association functional classification〉のI度(身体活動に制限がない心疾患患者)、およびⅣ度(いかなる身体活動を行うにも症状を伴う心疾患患者)に該当する者
14.65歳以下、80歳以上

1.Exacerbation of subjective symptoms of heart failure(dyspnea, fatigue, etc.) within the past week
2.Unstable angina or low threshold (induced by slow walking on level ground 2METs) myocardial ischemia
3.Severe valvular disease for which surgery is indicated, especially aortic stenosis
4.Severe left ventricular outflow tract stenosis (obstructive hypertrophic cardiomyopathy)
5.Untreated exercise-induced severe arrhythmia (ventricular fibrillation, persistent ventricular tachycardia)
6.Active myocarditis
7.Acute systemic disease or fever
8.Other diseases for which exercise therapy is contraindicated (moderate or higher aortic aneurysm, severe hypertension, thrombophlebitis, embolism within 2 weeks, serious other organ damage, etc.)
9.Patients with congenital cardiovascular disease
10.Renal failure with estimated glomerular filtration rate <30
11.Persons who have been diagnosed with psychiatric disorders
12.Malignant tumor
13.Persons who fall under the NYHA classification <New York Heart Association functional classification> grade I (patients with heart disease with no restrictions on physical activity) and grade IV (patients with heart disease who have symptoms in any physical activity)
14.65 years old or younger, 80 years old or older
