GLP-1受容体作動薬の適応があると主治医が判断した2型糖尿病患者のうち、研究の趣旨を理解し、文書による同意の得られた患者

Patients with type 2 diabetes who are judged by their physicians to be eligible for GLP-1 receptor agonists. In addition, patients who understand the purpose of the study and have given written consent.

除外基準/Key exclusion criteria

主な結果/Results

HbA1c，体重，体脂肪率はGLP-1受容体作動薬開始12か月後に有意に低下し，筋量および握力に有意な変化は認めなかった。栄養摂取量（炭水化物・たんぱく質・脂肪），肝逸脱酵素および総コレステロールも低下した。外発的摂食は3か月後に低下し12か月後まで持続した一方，情動的および抑制的摂食は一過性の変化にとどまった。さらに，外発的摂食スコアが高いほど体重減少量が大きかった。

HbA1c, body weight, and body fat decreased significantly at 12 months after initiation of GLP-1 receptor agonists, with no significant changes in muscle mass or handgrip strength. Energy intake (carbohydrate, protein, fat), liver enzymes, and total cholesterol also decreased. External eating decreased at 3 months and persisted to 12 months, whereas emotional and restrained eating showed only transient changes. Higher baseline external eating scores were associated with greater weight loss.

症例数は92例（男：女=54：38，58.1±13.7歳，罹病期間11.6±11.9年, HbA1c(%)8.2±1.6, 体重(kg)80.3±20.5, 体脂肪率(%)35.8±7.9）であった.
使用したGLP-1受容体作動薬は、経口セマグルチドが40人、注射セマグルチドが32人、デュラグルチドが12人、リラグルチド8人であった.

The study included 92 patients (male:female = 54:38; age, 58.1 +/- 13.7 years; disease duration, 11.6 +/- 11.9 years; HbA1c (%) 8.2 +/- 1.6; body weight (kg) 80.3 +/- 20.5; body fat (%) 35.8 +/- 7.9).
The GLP-1 receptor agonists used were oral semaglutide in 40 patients, subcutaneous semaglutide in 32 patients, dulaglutide in 12 patients, and liraglutide in 8 patients.

参加者の流れ/Participant flow

外来、入院問わず、新規にGLP-1受容体作動薬を開始した2型糖尿病の症例のうち、同意を得られた患者が参加した。
投与前、投与4週、12週、24週、52週後にそれぞれ血液検査、体重測定を行った。また、投与前、投与12週、52週後には体組成の測定と2種類のアンケート調査（食物摂取頻度調査、日本語版Dutch Eating Behavior Questionnaire）を行った。

Patients with type 2 diabetes who newly initiated GLP-1 receptor agonist therapy, regardless of outpatient or inpatient status, were enrolled after providing informed consent.
Blood tests and body weight measurements were performed at baseline and at 4, 12, 24, and 52 weeks after initiation. In addition, body composition measurements and two questionnaire assessments (a food frequency questionnaire and the Japanese version of the Dutch Eating Behavior Questionnaire) were conducted at baseline and at 12 and 52 weeks.
