目的/Objectives
目的1/Narrative objectives1	Modified thoracoabdominal nerves block perichondrial approach（M-TAPA）は腹部前面の幅広い効果範囲が期待できる体幹ブロックである。 しかしこのブロックのアプローチはエコービームとブロック針の刺入方向が成す角が鋭角となりやすく、ブロック針の視認性が悪くなりやすい。 一方、肋骨弓下transversus abdominis plane (TAP) ブロックや肋骨弓下斜角TAPブロックはエコービームとブロック針の刺入方向が成す角が鈍角となりやすいため、ブロック針の視認性は良い。しかし、広範囲の鎮痛効果を得るためにはプローブとブロック針を同時に動かし、局所麻酔を分割投与する必要性がある。 我々はsingle-bolus-shot 肋骨弓下TAPブロックを提案する。このブロックはリニアエコープローブを第9-10肋軟骨レベルで肋骨弓下に肋骨弓に沿って押し当て、内腹斜筋と腹横筋間の肋軟骨に近い位置で局所麻酔薬を単回投与する方法である。 Single-bolus-shot 肋骨弓下TAPブロックがM-TAPAと同等の鎮痛効果範囲を得ることができる場合、手技的に難易度の高いM-TAPAや肋骨弓下斜角TAPブロックに替わるブロックとなり得る。 本研究の主な目的はsingle-bolus-shot 肋骨弓下TAPブロックの効果範囲を検討することである。	The Modified thoracoabdominal nerves block perichondrial approach (M-TAPA) is a trunk block that can be expected to have a wide anesthetized area on the anterior surface of the abdomen. However, in this block, the angle formed by the ultrasonic beam and the block needle tends to be acute, and the visibility of the block needle on the ultrasonic screen tends to deteriorate. On the other hand, in subcostal transversus abdominis plane block (TAP) and oblique subcostal TAP block, the angle formed by the ultrasonic beam and the block needle tends to be obtuse, so the visibility of the block needle on the ultrasonic screen is better. However, these blocks require simultaneous movement of the transducer and block needle to administer local anesthesia for a wide anesthetized area. We propose a single-bolus-shot subcostal TAP block. In this block, the linear transducer is placed at the 9th-10th costal cartilage level along the costal arch, and local anesthesia is administered between the internal oblique muscle and transverse abdominis muscle near the costal cartilage as single dose. If single-bolus-shot subcostal TAP block achieves the same anesthetized area as M-TAPA, it can be an alternative block to M-TAPA or oblique subcostal TAP block. This is the pilot study to evaluate the anesthetized area of single-bolus-shot subcostal TAP block.

評価/Assessment
主要アウトカム評価項目/Primary outcomes	手術2時間後の麻酔領域（左右の前皮枝及び外側皮枝領域別に評価）	Anesthetized areas at 2 hours after surgery (The area innervated by anterior branch and lateral cutaneous branch are evaluated separately)
副次アウトカム評価項目/Key secondary outcomes	手術２時間後に咳をした際の疼痛スコア（NRS）、術後２４時間の頓用鎮痛薬要求回数、single-bolus-shot 肋骨弓下TAPブロックに関連した有害事象	Pain score (NRS) induced by coughing at 2 hours after surgery Additional analgesic requirements within 24 hours after surgery Adverse events associated with single-bolus-shot subcostal TAP block

選択基準/Key inclusion criteria	腹腔鏡下胆嚢摘出術をsingle-bolus-shot 肋骨弓下TAPブロックを併用した全身麻酔下で施行する患者	Laparoscopic cholecystectomy cases undergoing single-bolus-shot subcostal TAP block in addition to general anesthesia
除外基準/Key exclusion criteria	1. 体重45kg未満の者 2. BMI35以上の者 3. 開腹・開胸手術の既往がある者 4.正常なコミュニケーションが困難な者 5. 既存の神経障害がある者 6. 手術時間が６時間を超えた場合 7. 手術２時間後に意識が十分に回復していない者 8. 妊娠している者 9.局所麻酔にアレルギーがある者 10. その他施行者が不適切とみなした者	1.Body weight < 45 kg 2. BMI >= 35 3. Past history of thoracotomy or laparotomy 4. Communication difficulty 5. Existing neuropathy 6. Prolongation of surgical time ( > 6 hours) 7. Insufficient recovery of consciousness at 2 hours after surgery 8. Pregnancy 9. Allergy to local anesthetics 10. Other patients who are judged inappropriate by the investigators

その他関連情報/Other related information	脊髄神経前枝領域、および外側皮枝領域の効果を分けて評価するため、ピンプリックテストはそれぞれ以下のポイントで評価する。 前枝：体の中心線から5cm以内のライン 外側皮枝：腋窩中線ライン	To evaluate the efficacy of single-bolus-shot subcostal TAP block in the anterior branch and lateral cutaneous branch separately, pinprick tests are performed at the points below; Anterior branch: within 5 cm from the vertical line through umbilicus. Lateral cutaneous branch: mid-axillary line.
