目的/Objectives
目的1/Narrative objectives1	心不全再入院患者は初回心不全患者と比較して、長期予後は不良である。初回心不全患者と心不全再入院患者における、長期予後を規定する潜在的メカニズムの違いを明らかとすることを目的としている。	Patients with worsening o chronic Heart failure have worse prognosis compared to those with new-onset heart failure. The aim of this prospective cohort study is to clarify difference of underlying mechanisms associated with prognosis between patients with new-onset heart failure and worsening of chronic heart failure.

その他関連情報/Other related information	【スタディデザイン】 前向き観察研究 【登録患者】 急性心不全で入院した者 【観察項目】 既往歴 入院時バイタルサイン 入院時、退院時の体重、血液検査所見、心エコー検査所見、内服薬 入院時と退院時のフロセミド投与用量 舌圧測定、握力、下腿周囲径 バイオマーカー (BDNF) 【エンドポイント】 院内死亡の有無 退院後の全死亡、心血管死、心不全再入院の有無	Study design: Prospective cohort study Participants: Patients hospitalized with acute decompensated heart failure Observation: Background information Vital sign at admission and at discharge Laboratory test, echocardiography, medication including furosemide at admission and at discharge at admission and at discharge Sarcopenia test (grip strength, short physical performance battery, appendicular skeletal muscle mass by bio impedance analysis) Calf circumference Tongue pressure Biomarker (BDNF; brain-derived neurotrophic factor) Primary outcome: Composite endpoint including all-cause death and heart failure rehospitalization Secondary outcome: In-hospital death All-cause death Cardiac death Heart failure rehospitalization
