ヒトパピローマウイルス影響プロファイル(HIP)

Human Papillomavirus Impact Profile(HIP)は、女性におけるHPV関連健康状態の心理社会的影響を測定するためにデザインされた、妥当性が確認された自己投与質問表であり、0点(影響なし)～10点(最悪の影響)の離散視覚的アナログスケールを7つの仮定領域(心配および懸念、感情的影響、性的影響、自己像、パートナーの問題および伝達、医師との相互作用、およびコントロール-生活への影響)に分類して評価した29項目である。

項目スコアに基づいてDimensionスコアおよび総負荷スコアすべて0～100)を算出し,スコアが高いほどHPVの心理社会的影響が大きいことを示す。HIP質問表の日本語版は,翻訳,逆翻訳,比較,レビューおよび統合,ならびにMAPIによるフィールドパイロット試験を含む標準的な言語的妥当性確認プロセスによって作成された。

Human Papillomavirus Impact Profile (HIP)

Human Papillomavirus Impact Profile (HIP) is a validated, self-administered questionnaire, designed to measure the psychosocial impact of HPV-related health conditions in women. This is 29-item rated on a 0 (no impact) -10 point (worst impact) discretized visual analog scale grouped into seven hypothesized domains: Worries and concerns, Emotional impact, Sexual impact, Self-image, Partner issues and transmission, Interactions with doctors, and Control-life impact .
Dimension scores and a total burden score, all ranging from 0 to 100, are calculated based on item scores. Higher scores indicate a greater psychosocial impact of HPV. The Japanese version of the HIP questionnaire was generated through a standard linguistic validation process, which included translation, back translation, comparison, review and consolidation and field pilot testing by MAPI.

Kessler6Psychological Distress Scale(K6)は、6項目からなる簡易質問表であり、母集団調査における精神症状に関連する苦痛の程度及び程度を測定するためにデザインされたものである。K6は、世界中で臨床転帰の尺度として最も一般的かつ広範に使用されている自己報告ツールの1つであり、日本語版K6は既に開発され、スクリーニング性能は英語版とほぼ同等であることが示されている。

K6の心理的苦痛には以下の項目が含まれる:(1)あまりに悲しいので元気が出ない、(2)神経過敏、(3)落ち着きがないまたはそわそわしている、(4)絶望的、(5)すべてが努力であった、(6)価値のない。K6は回答の重み付けのない合計を用いてスコア化することができる-「常に1」、「ほとんどの場合2」、「一部の場合3」、「ほとんどの場合4」、「全くない場合5」。したがって、総スコアは0～24の範囲であり、K6スコアが0-7であれば低苦痛、8-12であれば中等度の苦痛、13～24であれば心理的苦痛のリスクが高いことを示す。K6を用いた過去の研究によると、日本では子宮頸癌スクリーニングの陽性結果が心理的苦痛と関連していた。

Kessler Psychological Distress Scale 6 (K6)

The Kessler 6 Psychological Distress Scale (K6) is a brief 6-item questionnaire designed to measure the level of distress and severity associated with psychological symptoms in population surveys. The K6 is one of the most commonly and extensively used self-report tools as a clinical outcome measure across the world. The Japanese version of the K6 has already been developed and demonstrated screening performances essentially equivalent to those of the original English versions.

K6 psychological distress includes the following items: (1) so sad nothing can cheer up, (2) nervous, (3) restless or fidgety, (4) hopeless, (5) everything was an effort, and (6) worthless. K6 can be scored using the unweighted sum of responses; 1 all of the time, 2 most of the time, 3 some of the time, 4 a little of the time, 5 none of the time. Thus, the total scores range from 0 to 24; K6 score of 0 to 7 indicates low distress, 8 to 12 moderate distress, and 13 to 24 high risk of psychological distress. Previous study using K6 described that a positive outcome of cervical cancer screening was associated with psychological distress in Japan.

HPV関連疾患(子宮頸部異常)群:
-スクリーニング時の過去6ヵ月以内にHPV関連子宮頸部異常の自己報告のために病院/クリニックを受診したと回答した被験者
-ICD-10診断コードにてHPV関連子宮頸部異常を有すると定義された被験者

非HPV関連疾患群:
-スクリーニング時の過去6ヵ月以内にHPV関連子宮頸部異常のため来院しなかったと回答した被験者
-スクリーニング時の過去6カ月以内に子宮頸癌のスクリーニングを受け、結果が正常であったと回答した被験者
-スクリーニング時の過去6ヵ月以内にHPV感染検査で陽性結果(陰性、記憶なし/不明、検査なし)以外の自己報告を行った者
-6ヵ月以内にICD-10診断/医学的補償コードによるHPV関連子宮頸部異常治療/診断を受けていない被験者

HPV感染群:
-スクリーニング時に過去6ヵ月以内にHPV関連の子宮頸部異常を自己報告しなかった被験者
-スクリーニング時の過去6ヵ月以内にHPV関連子宮頸部異常のため来院しなかった被験者
-スクリーニング時の過去6ヵ月以内にHPV感染検査陽性を自己申告した者
-6ヵ月以内にICD-10診断/医学的補償コードによるHPV関連子宮頸部異常治療/診断を受けていない被験者

For HPV related diseases (cervical abnormalities) group:
-Participants who answered they has visited hospital/clinic for self-reported HPV related cervical abnormalities within the past 6 months at the screening
-Participants who had HPV-related cervical abnormalities defined by ICD-10 diagnosis codes

For Non HPV related diseases group:
-Participants who answered not visit hospital/clinic for HPV related cervical related abnormalities within the past 6 months at the screening
-Participants who answered they received the cervical cancer screening with normal result within the past 6 months at the screening
-Participants who self-reported other than positive results (negative, don't remember/unknown and no test) in the HPV infectious test within the past 6 months at screening
-Participants who had no treatment/diagnosis of HPV-related cervical abnormalities by the ICD 10 diagnosis/medical compensation codes within 6 months

For HPV infection group:
-Participants who self-reported no HPV related cervical abnormalities within the past 6 months at the screening
-Participants who answered not visit hospital/clinic for HPV-related cervical related abnormalities within the past 6 months at the screening
-Participants who self-reported positive results in the HPV infectious test values within the past 6 months at screening
-Participants who had no treatment/diagnosis of HPV related cervical abnormalities by the ICD-10 diagnosis/medical compensation codes within 6 months

この調査結果は、HPV関連疾患群と非HPV関連疾患群の間で、総負担スコアおよび「心配と関心」、「感情的影響」、「性的影響」、「自己イメージ」などの領域スコアに統計的有意差があることを示しました。特に「感情的影響」と「心配と関心」の領域での差が顕著でした。HPV関連疾患群は、重症度に関係なく非HPV関連疾患群より高い心理社会的負担を感じており、年齢層別では20-29歳の若年層のスコアが最も高く、40-49歳の高齢層よりも高いことが確認されました。

The study showed significant differences in total burden and domain scores such as "Worry and Concern," "Emotional Impact," "Sexual Impact," and "Self-Image" between the HPV-related and non-HPV-related groups. Differences were most notable in "Emotional Impact" and "Worry and Concern." The HPV-related group felt higher psychosocial burden regardless of severity. Among age groups, the highest scores were in the 20-29 year-olds, exceeding those in the 40-49 year-olds.

以下は20歳から49歳の日本人女性のデータで、15万人がオンライン登録している「kencom」システムのデータベースから抽出されました。
対象者は3つのグループに分けられます。 (1) スクリーニング時に自己申告で「子宮頸部異常」があり、過去6ヶ月以内にHPV関連の子宮頸部異常の治療のために病院や診療所を受診した参加者(HPV関連疾患群の候補) (2) スクリーニング時に自己申告で「正常な細胞診」(つまり過去6ヶ月以内にHPV関連の子宮頸部異常の治療のために病院/診療所を受診しておらず)で、自己申告で過去6ヶ月以内に正常な細胞診検査結果があり、HPV感染検査が陽性ではない(陰性、覚えていない/不明、検査なし)参加者(非HPV関連疾患群の候補) (3) スクリーニング時に自己申告で「正常な細胞診」があるが、過去6ヶ月以内のHPV感染検査が陽性の参加者(HPV感染群の候補)
データは、オンライン調査での自己申告の性別と年齢データに基づいて抽出され、請求データで確認されました。請求データで性別と年齢が調査回答と矛盾する回答者は対象から除外されました。 対象の回答者数は、「子宮頸部異常」が450人、「正常な細胞診(細胞診検査による)」が300人でした。

Data of Japanese women aged 20-49 years were extracted from the database named "kencom" system in which 150,000 individuals registered online. The study population included three groups:
(1) participants with self-reported HPV-related "cervical abnormalities," who had visited hospitals or clinics to treat HPV-related cervical abnormalities within the past 6 months at screening (candidate for HPV-related diseases group),
(2) participants with self-reported "normal cytology" (i.e., no hospital/clinic visits to treat HPV-related cervical abnormalities within the past 6 months), with self-reported normal pap test results and not positive (other than positive, including negative, don't remember/unknown and no test) results in the HPV infectious test within the past 6 months at screening (candidate for non-HPV-related diseases group),
(3) participants with self-reported "normal cytology" and positive results in the HPV infectious test values within the past 6 months at screening (candidate for HPV-infection group).
The data were extracted based on the self-reported sex and age data on the online survey, which were confirmed with the claims data; the respondents whose claims data indicated sex and age contradicted from the survey response answers were excluded from the study population. The target numbers of respondents were 450 for "cervical abnormalities" and 300 for "normal cytology" (by pap test).

合計275,184通の招待メールが送付されました(第1回調査111,411通、第2回調査17,026通、第3回調査146,747通)。20歳から49歳の女性6,974人がスクリーニング質問票に回答しました。スクリーニングを通過し調査を完了した人のうち、639人が「子宮頸部異常」、289人が正常な細胞診と分類されました。除外基準に該当する人を除外した後、121人がHPV関連疾患群、283人が非HPV関連疾患群となりました。

In total, 275,184 invitation mails were sent (111,411 for the first survey, 17,026 for the second survey, and 146,747 for the third survey), 6,974 females aged 20 to 49 years responded to the screening questionnaire. Of those who passed screening and completed the survey, 639 were classified as having "cervical abnormalities" and 289 as having normal cytology. After excluding those who met the exclusion criteria, 121 participants and 283 participants comprised the HPV-related diseases group and the non-HPV-related diseases group, respectively.

Primary outcome: the HIP
The HIP is a self-administered disease specific questionnaire to measure the psychosocial impact of HPV-related health conditions in women. The HIP has 7 domains and 29 items under the domains. The HIP total burden score and seven domain scores each range from 0 to 100, with a higher score indicating a greater psychosocial impact of HPV-related disease.
Secondary outcome: the K6
The K6 is a 6-item questionnaire to measure the level of distress and severity associated with psychological symptoms in population surveys. The total scores range from 0 to 24; a K6 score of 0-7 indicates low, 8-12 moderate, and 13-24 high psychological distress levels.
